I do not work in a bank, but believe where I work we deal with insurance policies that have to do with Fidelity of staff of the bank. I've processed insurance claim where just 5 staff defrauded the bank to the tune of 700million within a period of 2years. I know Banks have internal audit, but believe me a lot of staff can also work their way around these systems.

For ur information, the distant relation actually invested 16million of the bank's money in the MMM scheme b4 it was frozen in December, and up till now he is yet to get back the money. When internal audit was conducted in December, his bosses had to source for the money and cover for him so they won't lose their jobs too cos how can they explain such amount was moved under their watch without their knowledge

I know a head of operations who took about 3m for quick business. Though don't know if it was from the ATM or vault. He was transferred to marketing pending investigation and was subsequently sacked.

The fraud was discovered within 3 days or thereabouts. But he had been doing it for long and returning the money.
