I have applied for ordinary reissue of driver's license for the past 4 months and I am yet to be issued. Who penalizes them for gross incompetence? The annoying thing is they keep telling me it's not their fault. They have the nerve to have such a list, it also not our fault that we defaulted!

I think this list is incomplete, it seems one sided. I believe it should reflect penalties for officials assaulting road users, officials demanding bribe.etc

It isn't easy to be 100% clean and off their sheet. No worries, they now collect bribes just like police, instead of having my car compound, or in their compound ; I'ld rather bribe.

The big question is: who issues the license (VIO, not FRSC) and who should enforce its violation (same VIO).
So why is Road safety dragging that job with the VIO?
IF the FRSC guys catch a defaulting driver, can they issue him a license?

I guess there should be proper job specification

Good info @op. But those Rd safety officials are unnecessarily ridiculous. Even if u have ur fire extinguisher, put ur seat belt on and obey all the rules. They will dig until they find something faulty in ur car. That's why one is better off not stopping for them @all if possible.
