It appears like a fixed drug reaction. Have you noticed it getting itchy, red, painful and more exuberant after taking a certain medication? If that's the case, you are allergic to the drug. Stop using it, after sometime perhaps a year, it will disappear. Another possibility is allergic dermatitis. A course of topical steroid will help.

There are many instances we can give, but the above gives you an idea. Check and see if there is a reason ,a factor, a habit, an activity that causes reoccurrence.

That looks like infection from the Tinea family.

First of all, go to the hospital or laboratory for skin culture and all necessary tests to rule out STD/STI

Once ruled out, go get these 3 things:

1. Mycoten Clotrimazole 2% (solution)

2. Gentian Violet paint (GV)

3. Hydrogen Peroxide

Like I said it isn't clear enough. It might also be a condition know as tinia versicolor

Go see a doctor that specialises in skin disease. They will carry out necessary test to ascertain the remote and immediate cause of your problem and administer correct dosage of medication....
