Go to your department with your first degree cert, waec ,birth cert,and nysc discharge cert for your screening.

Note that is after you must have paid the #4500 notification fee online to enable you print the screening form which is one of the documents you're to go with along with the aforementioned four. Your head of department will sign the screening form after verifying your documents, so will the HOD post graduate school.

Finally, you must go with cash because my department charged me 5k. I don't know about yours.

That's pure extortion, after paying the notification 4500 online. I never heard of screening fee my entire life in that institution, even as undergraduate then. This people �‍♂️
