Probably a thief while evading escape from location A jumped into an open manhole and crawled his way all the way to location B where there are no open manholes, only sealed manholes. Now the dude is trapped and losing sanity fast from being in a dark dirty confined space with poor ventilation.

Once he is rescued they should properly interrogate him on how he ended up there. He must be a criminal.
