Does it even mean that that organisation has no duty supervisor that would have dictated that the driver was drunk and unfit to drive?? They just gave him keys and off he goes to kill innocent people..

most definitely. Avoidable by the person involved. No matter how much punishment you pass for drinking and driving, people will still drink and drive.

That shit is a totally avoidable accident...in a developed countries, where there are intersection between a railway line and a road, when the train is 2000meters away, there will be an automatic barricade that will prevent motorists and pedestrians from crossing and alarm bells will be ringing to signal that a train was passing... When it passed, the barricade will automatically open again... Na so e be.
This is the first of it's kind. In Nigeria people drive recklessly and fail to adhere to traffic rules. I'm sure you're guilty of this. Some people see free space to pass one way inside heavy traffic, before you know it, they're there speeding up.
