This post is meant to clarify some issues surrounding menstrual flow-normal and abnormal.This is even made more important by the fact that sex education of adolescents in our part of the world by parents is considered a taboo,as a result,young adolescents are left to find their way through the ‘strange changes’their bodies go through during puberty and young adulthood.

A woman of reproductive age usually sheds the lining of her uterus(womb) once every month in cyclical pattern,provided she is not pregnant-this comes out as menstrual flow to the exterior.
Normal duration of a cycle is between 21-35 days lasting between 3-6 days for most women and is relatively fixed for every individual and differs from woman to woman.This shedding of the uterine lining (lining of the womb) may be accompanied by a slight cramp and discomfort that does not prevent you from going about your daily activities and you get relief when you take over the counter pain killers.However,when this is associated with severe pain that fails to respond to commonly used pain relievers, it then becomes a problem reffered to as dysmenorrhoea(severely painful menstrual flow).

Dysmenorrhoea; What is it?

It simply means “difficult or painful periods” and there are two types:primary and secondary.
Primary dysmenorrhoea is the most common type of dysmenorrhoea and symptoms include;cramping in the lower abdomen (belly)which may begin a day or two before your period and can last 2-4 days.

Secondary dysmenorrhoea is when cramps are as a result of medical conditions such as the presence of a tissue that is similar to the lining of the womb at another place in the body (endometriosis).This usually causes pain before and or during a woman’s period.

What causes this pain?

During menstruation,some chemical substances are released to cause uterine contractions especially in the early days.This may may cause nausea, diarrhoea and lightheadedness.

What are the symptoms of dysmenorrhoea?

Following pressure from different quarters in his country, Mugabe has surprised many by announcing his his refusal tor resign after 37 long years at the helm of affairs in the southern African nation. It will be recalled that members of the the ruling party earlier today sacked him as party chairman.

His wife Grace has been banished from Zanu-PF and his former Vice president Emmerson Mnangagwa has been appointed party leader.

Shortly after news of Sunday’s meeting emerged, a convoy left Mugabe’s official residence in Harare to the jeers of onlookers, although it was unclear if he was inside.
Marchers had massed outside his home on Saturday, revelling in their freedom to voice anger at decades of misrule as well as hope for a better future for Zimbabwe. Many waved the national flag, chanting and singing. Some embraced soldiers or posed with them for selfies. The march had been sanctioned by the military.

Euphoric crowds filled streets in the capital on Saturday morning and cars honked their horns calling for the veteran leader to step down.

“These are tears of joy,” said Frank Mutsindikwa, 34, holding aloft the Zimbabwean flag. “I’ve been waiting all my life for this day. Free at last. We are free at last.”

There were similar scenes in the southern city of Bulawayo, as well as abroad, where diaspora Zimbabweans held their own rallies.

I have seen several female patients in the gynae clinic presenting with what they term “toilet infection”,a term they use to describe a condition where they have one or more of the following symptoms;

1.Vaginal discharge

2.lower abdominal pain

3.vaginal pruritus

4.fever

A woman’s vagina (private part),normally releases secretions from glands situated therein,these secretions are not foul smelling,clear, not copious (unless during sexual stimulation, at the mid-cycle when ovulation occurs or during pregnancy)under normal circumstances and are not associated with the symptoms listed above.

When this discharge becomes foul smelling,cloudy,bloody,yellow or greenish and copius, an infection should be suspected.This can be caused by;

.Infective agents spread by unprotected sexual intercourse such as chlamydia,trichomonas and nesseria ghonorrhoeae.

.Vaginal yeast infection caused by fungus

.Normal bacteria that live in the vagina may overgrow (usually in people with lowered immune status like diabetics,elderly,people receiving cancer treatment)-this is called bacterial vaginosis.
The question now is;is it possible for a woman to get an infection from using a toilet?,the answer is yes;mostly public toilets in poor sanitary condition or any other shared toilet facility,other causes could be .......

Having a heavy,irregular menstrual flow not consistent with what you are used to can be a very unsettling symptom and many a patient present to the hospital with this complaint .
The female reproductive tract consists of he tract comprises from the exterior inwards of ,the vagina(private part),the uterus (womb) divided into the cervix and the main body, which is where the fetus (baby) lies during pregnancy, two tubes leave the upper pole of the body of the womb called fallopian tubes.These tubes end in enlarged finger-like projections called the ovaries which contain the woman’s eggs (or ova). Let me now go on to talk on what fibroids are.

What is uterine fibroid?

They are firm masses derived from the smooth muscle layer of the uterus (womb) ,non-cancerous (benign) in nature occurring in women of reproductive age group.They are the most frequently seen tumours of the female reproductive system.synonyms include; uterine myomas,leiomyomas or fibroma. It is said that 20-50% of women in reproductive age group have uterine fibroids though not all are symptomatic and not all are diagnosed

What causes fibroids?

Cause is not known but the consensus is that it arises from an aberrant smooth muscle cell(s) repondinding to the hormone oestrogen thereby multiplying rapidly to form fibroid nodules.

Girls may be unable to refuse unwanted sex or resist coerced sex, which tends to be unprotected.

SEX EDUCATION IS THE KEY.. We shall look into ways to prevent unwanted pregnancy in the next lecture
