Ity dee: I have swollen legs and sometimes palpitation in my heart. there is a big vein on my left shoulder but this is missing on my right shoulder. I have sharp pains on my right side and my urine is sometimes very yellowish and concentrated. i have done all test and came out clean but my legs are swollen and side painful and unable to sleep more than 4 hours every night. Pls help

please get kidney function tests done

Please think of acidity first. and anxiety. hardly any chances of this being cardiac at the age of 20!

I am back after quite some time. Sorry people for being absent, but was just busy with work and patients.

Let me point out, Troponin I is a test for a heart attack or a Myocardial Infarct. It has no value whatsoever in screening people with heart problems. It is to be used ONLY by doctors for the purpose of CONFIRMATION of a cardiac event. No use wasting money on it.

We are opening up one more centre in India, around 250 kilometres from the present one. Another piece of good news is that now we are taking patients from other countries as well. We have recently finished our 500th patient on EECP successfully, and we are proud to have helped 499 out of 500 patients avoid an expensive and risky Bypass Surgery!

My sincere thanks to all my patients who have contributed to our cause of helping heart patients avoid unnecessary surgeries and stents!

this sounds like Angina. You need to be investigated further. An ECG and Echocardiography would be helpful.

Baroba: Hi Dr Sandhu, a bp no of 120/89 is it borderline hypertensive, secondly I engage in extreme cardio exercise whereby my HR shoots up to 188 and then drops, are there any dangers associated with this sort of exercise. Am worried of any long term damage this might cause to the heart. My father died of myocardial infarction last year aged 65..

second question please, my father in law has been diagnosed with CHF, he is suffering from SOB, peripheral oedma,extreme exhaustion, and vomiting. He is 60yrs old, he is presently been treated by a renowned cardiologist in Enugu. I just need to know what the prognosis are, can it be cured or just managed. Thanks for your help Dr.

Considering your family history of Heart Disease, you should engage in regular exercise but not in extreme cardio workouts. It can cause unnecessary stress to your Cardiovascular System, which can cause serious problems in the future.

In case of your father in law, CHF needs to be treated at the earliest. I recommend ECP(External Counter Pulsation) for the treatment of CHF. This treatment is approved by USFDA for the same. It results in very good improvement in the patients symptomatically, and the medicines also can be reduced. I have posted some details about this treatment earlier as well.

Until later dat they told me is ulcer which i av been told to stop drinking and all stuffs. But, i'm experiencing chest pain and heat,back pain and heat as if there is wound at my back down to my waist/pelvic girdle.

Please get a HRCT Chest done to rule out any abnormality of the Aorta(Aneurysm etc).
Also, please ask your physician if he could consider any problem with the pancreas.

It is not easy to guide such a case from so far. I will try to provide the best guidance but you will need a physician locally to help you pin point the exact problem and rectify it.

please drink ample quantity of water and limit your salt intake. also decrease the amount of fried stuff in your diet. and please get into mild exercise like walking for 30 minutes everyday.

hello doc. early in the year I started having pains in my heart that some times could be felt in my left arm and neck,I went to see the doctor who prescribed drugs including an anti inflammatory drug.I felt better for some time and it came back.the past few months has been hell.I don't feel it in my arms like before but I have been feeling the pain in my heart.I am 25,I don't drink,don't smoke and I am not fat.I am slim.I need ur advice

what you describe here is typically chest pain due to a cardiac(heart related) cause. the anti inflammatory drugs are not going to resolve your problem. as per my honest opinion, you should be treated conservatively(without any surgical method).

the treatment of choice for a young patient like you is EECP/ECP(external counter pulsation), which is a machine based treatment for Angina(chest pain). It is a USFDA approved treatment for the purpose. I am not sure whether it is available in Nigeria or not, but in India we have around 200 or so centers providing this service. Any further questions about the treatment, I will be happy to answer them!

the surgical options involve stents and bypass surgery, which i would not like to consider in such a young patient. they are not only more expensive, they are invasive and carry quite a fair bit of complications.

Excessive drinking can cause high BP. Follow some basic steps to reduce your BP

1. Drink 3 liters of water every day
2. Reduce your salt intake
3. Reduce fried stuff in what you eat
4. No Smoking

Please avoid all strenous activities till you feel better. Any physical ctivity that strains you can potentially cause hyperacidity, and is best avoided.

Please avoid the painkillers. Take some antacids first!
Maggi is good for once in a while...but if you eat it regularly, it can potentially cause acidity and constipation.

drink plenty of water and eat some yoghurt/curd everyday for 2 weeks.

this could be Angina. ECG only shows changes which have occurred, not the ones which could happen!

pardon my ignorance, but i completely missed the point.

what exactly does this information do to help anyone?

Actual facts

1. Type 1 Diabetes effects between 1 to 17 people per 100000, varying with the country where the incidence has been recorded
2. It is known to be of autoimmune origin, exact etiology unclear
3. it can be distinguished from Type 2 Diabetes by C Peptide measurement(to measure the production of Insluin in the body)
4. It is treatable by Insulin, which is life long because essentially, the problem is the lack of Insulin production in the body
5. Pancreatic transplants are useful in extreme cases. Stem Cells are being used in a few locations as an experimental modality.

I dont know where you got this list of things which can go wrong...but apparently most medical schools teach doctors to understand the common diseases much better as compared to rare diagnoses which might never be seen in 40 years of work!

I hope this information improves the understanding of the disease for everyone on this forum.

You should get a chest X Ray ASAP, the features you describe could possibly be Pneumonia or LRTI with Pleurisy. You will need the right antibiotics for 2 weeks minimum, alongwith other medicines.

ECG and Echo can only detect an existing anomaly, but not the stress on the cardiovascular system. the symptoms you describe point towards a possible circulatory impairment of the heart. although moderate level of gastritis can also produce these symptome. please take some anti acidity syrup for 5 days. if the symptome persist, you may need to undergo Angiography or a stress test to investigate further.

what you describing in your symptoms do indeed point towards a cardiac cause. the pain you feel on exertion is called Angina. It has to be investigated regarding what the exact problem maybe, but the first step would be getting some investigations done from a cardiologist.

In India, I would have ordered an ECG, Echocardiography and some Blood tests. I am not familiar with the system followed in Nigeria, but i am sure they will also get the same tests done and then decide on the future course of action.
At our medical center in India, we use the latest non surgical methods for treating heart disease like ECP(External Counter Pulsation), ACT(Artery Clearance Therapy) and other methods.

You should contact a cardiologist as early as possible. I hope you get better and join the military!

Rapid Heart Beat is technically called tachycardia. It can be due to a variety of causes. To determine the cause and future course of treatment, one has to be investigated for the exact diagnosis.
Please get an ECG done and consult a local Cardiologist.
