The form is N5000, you can submit it at the immigration headquarter in Abuja after filling it. They will measure you and if you are qualified,u will be given ur letter of employment immediately. I already collected mine.

For me I printed another slip after I edited my application, nothing change, it is still the same Application number,except the Transaction number that change.
