He received recessive genes from both parents. If you know genetics you will know a recessive gene will manifest most times if you have a pair of it from your parents. A mutant gene can cause color blindness, can cause hardened fingernails in humans, can cause albinism in humans even and a host of other congenital anomalies. So yes a single mutant gene can affect Thanos the way it did.

Same way a deviant with an eternal gene due to heritance of a recessive eternal gene from both deviant parents is still a deviant.

If one of his parents were deviants then 50% of Thanos would be a deviant meaning he would be half deviant and half eternal. Thanos is over 90% an eternal boss. And also this is just a trailer, a clearer picture will be painted when it comes out.
