You'll need your national id card and the information on it must correspond with intending data on the passport. cost will depend on the data you're changing.

I don't work there but I can give you the lady's number that help me, so when you go there you won't find it hard.

it's a big lie! You can only make such change of data at Hqt, I just finished doing mine, though it took 2weeks and 3days.
