1. My child bears her father's name on her Birth certificate. So I declared I was single on my applicant.
I got my baby daddy to sign a letter authorizing me to travel with my child (Letter of Consent). I also notarised this
2. I downloaded another consent form on the CIC site that her father filled and also signed and notarised
3. I attached his intl passport.
4. There's no harm in the childbearing your surname. U can put her father's name on the Birth Certificate
5. Try and be on good terms with him to get him to fill the above docs because it is highly necessary for this journey.
