Yes, they are applying with me (my spouse and two children less than 3 yrs). Where will I add them as dependents? Are they required to pay applications fees? If yes, how do I book their appointments with my email address?

Are they applying with you? If yes add them as dependants. If no then you have to open a new profile. Use the senior one as major applicant and the other as dependent. Note: that you can't use an email already attached to your profile. Set up a new email.
