Cutesthunk: Oga Labo, the screen form u printed, was it just a coloured copy then having to make phocopies of it? And was it so so important that you come with ur state of origin cert.? Cuz a handful of guys i knw dat are already studnts dre said its nt necessary cuz the claimed state on my jamb slip is nt my state, na mistake state, tel me hw it ws, THANKS.......

For the screening form,itz all coloured(4) and avoid making mistakes on it cos some guyz are thr @ d locatn giving to you,ready to use u to make money.similar case happened to me to.

If it's GCE,you need statement of result or if the cert don ripe,waka go collect ham sharperly.
