The term "family planning" is often used as a synonym for "birth control." Family planning, however, does not only involve contraception. Family planning also takes into account planning your child's birth for specific times (possibly by spacing births a few years apart from one another) and planning for a child when you have challenges conceiving one.

Significance
Family planning is important for the health of a mother and her children, as well as the family's economic situation. According to the United States Agency for International Development, having children more than five years or less than two years apart can cause both a mother and her children serious health consequences. The financial consequence of having children involves the medical costs of pregnancy and birth and the high costs associated with actually bringing up children. Since parents are responsible for providing education, shelter, clothing and food for their children, family planning has an important long-term impact on the financial situation of any family.

Types
Family planning includes contraceptives, sexuality education and natural family planning techniques. Contraceptives include barrier methods like diaphragms, condoms and hormonal birth control. It can also include surgery (hysterectomies for women or vasectomies for men) or intrauterine devices (worn inside a woman's vagina during intercourse). Sex education assists families by teaching youth how their reproductive systems function, how to use contraceptives and the truth about pregnancy and birth control. Natural family planning techniques teach women and their sexual partners how to chart fertility cycles. Couples can abstain from having sex or use a barrier protection method during fertile times to prevent pregnancies. Learning about a woman's fertility cycles can also benefit couples who are having challenges with desired conception.

Ovarian cysts are closed, sac-like structures within the ovary that are filled with a liquid or semisolid substance.
Ovarian cysts may not cause signs or symptoms. Larger cysts are more likely to cause signs and symptoms such as:
Pain in the abdomen, pelvis, sometimes radiating to the low back, is the most common symptom
Feeling of bloating or indigestion
Increased abdominal girth
Feeling an urge to have a bowel movement or having difficult, painful bowel movements
Pain during sexual intercourse (dyspareunia)
Pain in the lower right or left quadrant of the abdomen on one side
Nausea and vomiting

Most ovarian cysts are diagnosed with ultrasound or physical examination. Transvaginal ultrasound is a common way to examine ovarian cysts.

Rupture of an ovarian cyst is a complication that sometimes produces severe pain and internal bleeding. A ruptured (burst) ovarian cyst usually causes pain on one side that comes on suddenly.

Pelvic pain is more common in women. There are many different causes of pain in your pelvis. They can be separated by when they tend to happen, if you are pregnant and if they are accompanied by other symptoms such as vaginal bleeding. The most common causes are mentioned below. Most will improve with painkillers. Most recurring causes have treatments available.

The pelvis is the lowest part of your tummy (abdomen). Organs in your pelvis include your bowel, bladder, womb (uterus) and ovaries. Pelvic pain usually means pain that starts from one of these organs. In some cases the pain comes from your pelvic bones that lie next to these organs, or from nearby muscles, nerves, blood vessels or joints. So, there are many causes of pelvic pain.

Pelvic pain is more common in women than in men. This leaflet will deal with the most common causes of pelvic pain in women.

Pelvic pain can be acute or chronic. Acute means that it is the first time you have had this type of pain. Chronic means that pain has been a problem for a long time - more than six months.


What are the causes of pregnancy-related pelvic pain?

Miscarriage: miscarriage is the loss of a pregnancy at any time up to the 24th week. 7 or 8 miscarriages out of 10 occur before 13 weeks of pregnancy. The usual symptoms of miscarriage are vaginal bleeding and lower tummy (abdominal) or pelvic cramps. You may then pass some tissue from the vagina, which often looks like a blood clot. Ectopic pregnancy: an ectopic pregnancy is a pregnancy that tries to develop outside the womb (uterus). Usual symptoms include pain on one side of the lower abdomen or pelvis. It may develop sharply, or may slowly become worse over several days. It can become severe. Vaginal bleeding often occurs, but not always. It is often darker-coloured than the bleeding of a period. [b]Rupture of corpus luteum cyst
: a corpus luteum makes hormones that help keep you pregnant, until other organs such as the placenta take over. It forms after the release of the egg at ovulation. They are often found, by chance, when you have an ultrasound scan for whatever reason. They often cause no problems at all and clear up without treatment. Sometimes it can become too swollen and may burst. This may cause sharp pain on one side of your pelvis. If you have pain in your pelvis in the first 12 weeks of your pregnancy, see your doctor.

Premature labou[/b]r: normally labour starts after 37 completed weeks of pregnancy. Normal labour usually starts as tightenings felt across the lower abdomen. These become stronger, more painful and closer together. You may also have a 'show'. This is the mucous plug from the neck of the womb (cervix). If you have a gush of fluid from the vagina, your waters may have broken. You should contact your midwife immediately. If you have pelvic pains that come and go in a regular pattern, contact your midwife for advice. See the separate leaflet called Premature Labour for more details.

[b]Placental abruption
: rarely (about 6 times in every 1,000 deliveries), the placenta detaches from the wall of the womb. Before 24 weeks of pregnancy this is a miscarriage; however, after 24 weeks it is called an abruption. When it happens it is an emergency. This is because the baby relies on the placenta for food and oxygen. Without a working placenta, the baby will die. The staff in the maternity department will quickly try to deliver the baby. This is usually by emergency caesarean section.

Vaginal bleeding can occur anytime in pregnancy. It is relatively common, and can occur during any trimester from conception (when the egg is fertilized) till the end of the pregnancy. Vaginal bleeding during pregnancy happens to about 15–25% of pregnant women especially during the first trimester (first 14 weeks) of pregnancy.
However, bleeding can also occur in the second and third trimester of pregnancy , so it is important to know the sign of a possible complication and get checked by an obstetrician gynaecologist to make sure you and your baby are in safe hands.
Abnormal vaginal bleeding in pregnancy is defined as bleeding during pregnancy that is usually as heavy as the bleeding you have on the second day of your menstrual period.
Causes of vaginal bleeding during early pregnancy (first trimester)
Bleeding in early pregnancy can be caused by some minor problem such as;
• Sexual intercourse in pregnancy
• Infection of the cervix, vagina or a sexually transmitted infection (such as chlamydia, gonorrhoea, or herpes)
• Implantation of a fertilized egg in the lining of the uterus which happens around the time of the menstrual period. Implantation bleeding may occur before a woman realizes she is pregnant.
• Changes in hormone
• Cervical changes
Major or severe causes of vaginal bleeding in early pregnancies includes miscarriage or early pregnancy loss, threatened miscarriage and ectopic pregnancy.
Miscarriage or early pregnancy loss is the most common cause of vaginal bleeding which occurs during the first 12 weeks of pregnancy. It does not necessarily mean the pregnancy will be lost or miscarried. If a miscarriage occurs with the current pregnancy, most women will go on to have a future healthy pregnancy. Signs of Miscarriage include
• Vaginal bleeding
• Cramping pain felt in the lower abdomen (stronger than menstrual cramps)
• Tissue passing through the cervix and vagina
• Back pain
Threatened miscarriage occurs as a result of abnormal bleeding and abdominal pain that occurs while the pregnancy continues, anything other than spotting during the first trimester may be considered a threatened miscarriage. i.e. spontaneous pregnancy loss. Symptoms of a threatened miscarriage are bleeding and mild cramping, but the cervix is closed, and the foetus survives.
Ectopic pregnancies are less common than miscarriages, but potentially dangerous and life-threatening, fertilized embryo is implanted outside the uterus, located within one of the fallopian tubes. The fallopian tube accounts for most ectopic pregnancies. this sometimes is referred to as a tubal pregnancy, which means that as the pregnancy grows and the tube distends, abdominal pain becomes increasingly severe and as a result of this, sometimes the pregnancies can rupture the Fallopian tube, leading to major blood loss and in most cases a blood transfusion and emergency surgery are needed. Major signs and symptoms of ectopic pregnancy are;
• Cramping pain felt in the lower abdomen (usually stronger than menstrual cramps)
• Low levels of HCG
• Abdominal pain
• Absence of menstrual periods
• Spotting (vaginal bleeding or intermittent bleeding)
• Light-headedness or dizziness
Women with higher risk may have had:
• An infection in the tubes
• A previous ectopic pregnancy
• Previous pelvic surgery
Causes of vaginal bleeding during pregnancy (second and third trimesters)
Bleeding or spotting in second and third trimester can be as a result of;
• Sexual intercourse or having a pelvic examination internally by your OB/GYN or midwife
• Cervical problems such as cervical insufficiency or incompetency (when the cervix opens too early in pregnancy) and
• cervical infection
Major or severe causes of bleeding in later pregnancy include
• Placenta previa
• Preterm labour
• Uterine rupture
• Placental abruption.

It is appropriate and advisable to call your OB-GYN doctor if abnormal vaginal bleeding is noticed. It is particularly important to seek medical attention if the bleeding is heavy (like a menstrual period), becomes painful or crampy.
