KNEE OSTEOARTHRITIS

Osteoarthritis is the wearing down of joint cartilage and the underlying bone. It is the commonest disease of joint worldwide with knee osteoarthritis being the most common. Osteoarthritis is the degeneration of joint cartilage and the underlying bone. Mostly affects people above the 45yrs. Women more affected than men.

The knee is the largest and strongest joint in the body. It is made of the lower end of the thigh bone, the upper end of the shin bone and the knee cap. The ends of these bones are covered by cartilage, a slippery tissue which provides a smooth surface for joint motion. Cartilage also acts as a cushion between the bones.

It can broadly be classified as primary (no known cause) and secondary (cause of the articular degeneration is known).
Risk factors for knee osteoarthritis include:
1) Age: ability of cartilage to heal itself decreases with aging.
2) Obesity weight increases the load on the joints especially the knee joints.
3) Gender women more at risk than men especially above 55yrs. This may be due to changes in hormonal levels after menopause.
4) Trauma knee injuries sustained during sports or accidents predisposes to osteoarthritis later in life. Osteoarthritis in this group tends to occur earlier and with more severe symptoms.
5) Repetitive stress injuries minor trauma which ordinarily would not cause injury, when it occurs repetitively may accumulate to cause serious injury to the knee, leading to osteoarthritis subsequently.
6) Bone deformities Genu valgum (k –leg) or Genu varum (bow leg)

Patients with knee osteoarthritis may present with any of this symptoms:
°Pain during or after movements
°Stiffness: commonly after awakening or after a period of inactivity.
°Grating sensation may be felt on moving the knee. Popping or crackling sound may be heard.
° Swelling around the knee, this may be constant or activity related.

Knee osteoarthritis is a clinical diagnosis and but be confirmed by relevant radiographs. Other investigations may be requested to identify predisposing factors.
Treatment can be non surgical or surgical.
Non surgical treatment is usually the first line treatment. It can be non pharmacologic or pharmacologic(use of medications) treatment.
Non pharmacologic treatment include
• Lifestyle modification
°Minimize activities that aggravates the condition e.g climbing stairs
°Avoid high impart activities e.g jogging or tennis. Swimming or cycling will put less stress on the knee.
°Losing weight reduces stress on the knee resulting in less pain.
• Physiotherapy
Helps increases range of motion and flexibility and also help strengthen the muscles around the knee.
°Assistive device
Using cane, wearing a brace can be helpful by taking off load from the knee or redirecting load passing through the knee.
°Application of heat or ice may soothe acute pain
° Elastic bandage: provide support to the knee.
Pharmacologic treatment
There are many arrays of medications available to manage arthritis. These include analgesics, anti inflammatory agents and other medications which may alter the viscosity of the joint.
Surgical treatment:
Usually a last resort if conservative measures fails or is no longer effective. Surgery can relieve pain, correct deformity and improve range of knee motion.

Welcome to the concluding part of caring for the back. The importance of maintaining good posture cannot be overemphasized.
General tips on how to maintain good standing, sitting, sleeping, walking, lifting and driving postures were discussed.
Today we will look at how weight and exercises affect the back. We will also consider what “working smart” means with respect to protecting the back.

Maintain a healthy weight

°  Avoid overweight, obesity or rapidly gaining significant weight.

°  Excess weight, especially belly fat adds extra stress on the muscles, ligaments and tendons of the lower back.

Regular exercises

°  Staying active and keeping your body moving helps maintain a healthy spine.

°  Best exercises for the back and neck combines stretching, strengthening and aerobic activity.

Bonus: Work Smart

°  Choose a chair that provides lower back support.

°  Your knees should be at about the same level with your hips (900) and your feet should rest comfortably on the floor.

°  Use a chair with lower back support or place a pillow behind the back.

°  Ensure your forearm and hands are in a straight line with your desk.

°  Have the top of your computer screen at eye level.

°  Avoid answering a phone call with the phone cradled between your ear and shoulder to avoid neck pain. Instead hold the phone to your ear or use a headset.

°  Plan regular breaks to periodically get up and walk around the office or home.

In continuation of our series on caring for the back, we will be looking at correct lifting and driving postures.
We have all found ourselves in situations where we have to do some lifting, either at work or at home. It is important to know the correct posture for lifting in order to protect the back.
5) Correct lifting posture
° Do not try to lift objects that are awkward or heavy
° Make sure you have firm footing.
° For objects lower than the level of your waist, bend at your knees and hips, keep your back straight.
° Stand with a wide stance close to the object you want to pick up and keep your feet firm on the ground. Bend your knees such that your arms are at the same height as the item.
° Lift the object using your leg muscles. Straighten your knee in a steady motion. Lift objects only chest high.
° Avoid jerking the object up to your body.
° If possible, instead of lifting, carefully push or pull the object.
° Get help if the load is heavy.

6) Correct posture while driving
° Move the car seat forward to keep knees bent and higher than the hips.
° Sit up straight
° Sit close enough to the wheel so that you don’t have to stretch for the pedals.
° Move the back seat as far as possible and hold onto the side of the car, back of the seat, or even the dashboard for additional support when getting in or out of the car.
° Bend at the knee and hips and try not to twist at the waist when getting in and out of the car.
° Avoid twisting to reach for objects in the back seat.
Caring for the back must be intentional and continuous, to keep the back safe at all times.
