This is quite interesting but lets try and look at it from the other side. Most of the places mentioned are lacking in structure - people doing defined activities. Registering at a hospital is not supposed to be the work of the nurse. Nurses are meant to take your vital signs and then put it on your folder which must have been brought to them by the administrative staff. The doctors are waiting in the consulting room for such folders to be moved to them. The doctors by the way should be competent and mature people who should be able to douse any charged atmosphere instead of adding fire.
