Your medical fitness certificate is still valid since it's not up to three months yet.
For casual clothes, go with 2-3 for Sundays and Fridays depending on your faith.
It's not compulsory to go with bucket, you can buy there.

Hope you know you can’t charge in the building? You have to go to mammi market, pay your N50 everytime you wanna charge
So there’s no need for extension. Just your chargers

I think the form will be available again when call up letter is out.. I filled mine since March but wasn't deployed with stream 1.. According to the info from NYSC,we have to fill the form when the call up letter is available

Yes; either one from a government or military hospital is accepted.
