go to call settings then proximity sensor then deactivate it.

remove your screen guard

I had the same issue. You need to vigorously clean the area around your phone's sensor. Dirt is blocking it and it must be cleaned or this will keep happening.

Use alcohol or wet cloth to scrub it and allow it to dry. If it keeps happening, change the screen guard (remember to tell the person changing it to help you clean the sensor area very well).
