1.Yes I did. It was addressed to the PCO Alausa Passport office, Ikeja Lagos. The business centre around the office can help you do it. It must be typed. And the child needs consent letter from the mom and dad.

2. Yes I did. But I suggest that as early as 7am/8am you're there. This is because the documents have to be verified at different desks, before it will finally be approved for capturing.

We were called in at about 4.15pm to seat on the queue for capturing. At 6pm, I was captured. Left there at 6.20pm.
I spent approximately 6hrs there
Collection date is on the 8th of March.

Below are documents to go with:
1. Online application form
2. Payment slip
3. Appointment slip
4. State of Origin Certificate (Photocopy and Original)
5. National ID card/NIMC Slip (Photocopy and Original )
6. Age Declaration ( for those born in the 80's) OR Birth Certificate from National Population Census (for those born in the 90's and 20's) - Photocopy and Original.
7. Guarantor's form and Data page of your guarantor. The guarantor's form can be notarized inside the building for N500.
8. Two passport photograph of yourself
9. One passport photograph of your guarantor.

It can't be gotten online. They made it readily available to those using an officer to process their passport. Don't know if this is applicable to other centres.

You have to make extra payments to receive the booklet in time. The demand is massive, so the Immigration guys are making cash off the whole situation.

If an individual lost his or she passport, he or she can get it back. First you need the Previous lost passport number...(Very Important)

Secondly you do a police extract stating that u lost ur passport

Thirdly you apply for another passport (Investigation will be done) before ur passport will be reissued
