Affidavit and Declaration of age is the same jor. And FUNAAB will not accept anything other than "National Population Census" Birth certificate for registration

Ok I av checked d one with me it is d NPC Certificate of Birth but I av an issue with it cos there is a different third name on it,different 4rm d third name I used for other documents but I av a declaration of age

Declaration of Age is different from Name.
It maybe accepted or not, I have no idea. But I would suggest you get another one, but if you are gambler, you can give it a try, if rejected... It's so fast and easy to get a new one.
