Not every school will agree to mail your initial i20 to an address other than one in the applicant's home country. I tried with UMass Lowell and the international student adviser told me it's against the policy of the school to do so. So you should ask if it's allowed. If not, I guess you'll have to use eshipglobal

I paid about $110. I used FedEx which was the cheapest. Dhl was much higher.

I don't think there is any format expect the big bosses here have one. But I could share some tips. 1) Take the standardized tests GRE & TOEFL get good scores. 2) check school ranking and program ranking 3) check admission policy e.g No advisor with funding No admission 4) Check test scores and GPA requirements 5) Check priority deadline 6) Mail to ask for preferred test scores or average test scores from the previously admitted class 7) write a standard Sop, Cv and make your REFEREES normal human being.

At some point you'll know when things are going your, maybe if the program has a departmental pre application form and they give you the nod to apply or you apply for a research position, conduct a video or phone interview with the prof and he gives you a nod, or at some point during application you get an interview.

I think $110 is quite high for fedex. I paid $50 for the same fedex to ship directly to me. Maybe the price varies with schools.
