There are many kids like this born with medical abnormality called CONGENITAL AMBIGUOUS Instruments. At birth you cannot say whether they are male OR female. Some smart parents would give such kids neutral / unisex names like Joy, Nkem, Damola, etc until they go through all the laboratory investigations to determine their definitive sex. AND THE ONLY DEFINITIVE LABORATORY INVESTIGATION REQIURED HERE IS CHROMOSOMAL KARYOTYPING!
Children born to ignorant families grow up like that. And later in life they are faced with many confusions about which sex role to assume. Because of the excess androgen, a female would look like a male ( with male-hair pattern called hirsutism, enlaraged clitoris mistaken for penis, exaggerated labia majora mistaken for scrotum, usually empty without any testes, deep male-like voice, etc). And the affected male has low level of testosterone thereby appearing like a female ( small penis called micro-penis mistaken for clitoris, thin voice, enlarged breast- if plagued with high estrogen level.)
Some of these kids grow up in such confusion. The confusion is worsened when they found out they are not physiologically suitable to perform the sexual duty of their assumed sex role. Then, the quest for laboratory investigation comes. Then the need for corrective surgery. And because many people don't have the knowledge of this thing called ambiguous Instruments, the victims capitalizes on that and begin to talk of transgender in order to make it socially relevant.
My grouse with madam Sade Adu is that, by virtue of her status and exposure, this surgery was supposed to have been done at early age 1-2 years of life.
