In as much as I do not like criticizing people for any step they take(I believe there are always some logical reasons behind every decision made no matter how foolish it may seem)...I still do not understand why he'd want to look like a tiger and still expect people t swamp around him like he's a place of adventure or a tourist centre

As long as he's deemed to have capacity, the surgery will go on regardless of however poor the request may be adjudged to be.. Otherwise, you might be seen as discriminating against him.
Possible exit point could be to send /request a 2nd opinion.
