has been made and according to him he has submitted d form. i asked him

Please, I am about to submit my application and my son had a birth injury (called ERB's palsy) at birth, which affected the nerves to his left arm and makes it difficult for him to raise it as high as his right arm. He requires occasional physiotherapy to strengthen some of his left shoulder muscles. Otherwise, he is healthy in every other regard.

To be safe, answer yes to the medical question and explain in the space under that you do not consider his condition to be serious and you believe it would not burden their health system.

IME means Immigration Medical Exam. So, IME number is displayed on an IME form which is sent by IRCC. To the best of my knowledge, no one receives that form except they've been asked to redo medicals by IRCC. So, leave it blank. IOM knows what to do.

Yes. Old Work place: Reference letter + bank statement highlighting the deposits
New workplace: Reference letter + payslips (employment is extra, add it if you like)
