The University College Hospital (UCH), Ibadan, Oyo state, is the first teaching hospital in Nigeria, and it lacks a basic diagnostic machine: the magnetic resonance imaging (MRI) machine. This has put a strain on patients who require proper diagnosis and treatment.

Being a radiologist, one would expect getting an MRI investigation done to be a walk in the park. This is not the case for Ademola Joseph, a 37-year-old radiologist and father of two.

Joseph has a towering figure but is now slightly bending due to his knee injury during a football match last August. He walks with an uncomfortable limp, making occasional grimaces. He was visibly in pain as he interacted with the reporter.

“After the injury, my doctor recommended an MRI investigation before he could start treatment. The test is important for him to see clearly what might have gone wrong there,” he said, pointing to the affected knee.

After the injury, his work and personal engagements suffered remarkably, just like his knee.

The MRI machine in this hospital (UCH) has been out of use for several years; so, the only alternative I have is to get the test done in a private facility. But it is so expensive that it would require me to take a loan. Before now, it was a matter of simply walking in and having the test done at UCH,” he said.

Joseph has been a radiologist for almost a decade. He feels betrayed by the healthcare system, which cannot take care of his medical needs despite his diligent service.

An MRI machine is critical in the diagnosis of certain conditions, such as injuries, tumours, cysts, certain heart problems, and more.

It uses strong magnetic fields and radio waves to create detailed and clear images of the organs and tissues within the body.

“It is one of the most advanced forms of medical investigation,” says Godwin Ogbole, a professor and head of the radiology department at UCH.

THE PREMIER HOSPITAL

The University College Hospital, a foremost tertiary health institution in Nigeria, was established in August 1952. The hospital was launched to close the training gap of medical healthcare professionals in Nigeria and Sub-Saharan Africa.

The MRI centre, located in the south-east wing of the hospital, was inaugurated by former President Olusegun Obasanjo in 2005. But it only worked for eight years.

In 2014, another MRI machine was inaugurated by Temitope Alonge, the serving chief medical director. It was a private-public partnership between ERS Nigeria Limited, a Lagos-based medical supplier, and the UCH.

It stopped working in 2019.

While there are more advanced MRI machines depending on power, the machine in UCH is 0.2T (Tesla is the standard for measuring the magnetic field strength of a machine).

Globally, a basic MRI scanner costs $150,000 but can exceed several million dollars. Research showed that Japan has the most MRI scanners per capita, with 48 machines for every 100,000 people.

“Currently, about 80% of Africa’s population does not have access to MRI. Even if they can afford it, the service is not available,” Ogbole said.

He noted that over 10 countries in Africa do not have a single MRI and the 1.5 Tesla MRI machines in Nigeria are concentrated mostly among the top 5% of the population who live in urban areas like Lagos, Abuja, and Port Harcourt.

Patients now have to look for means to carry out MRI investigations in private facilities, which are usually more expensive than similar services offered at UCH when the machines were active. For others who cannot afford private services, they are forced to live with their conditions.

Adeola Adebisi had to suspend her compulsory national youth service for over six months. She has been battling with back pain for five years. She was diagnosed with scoliosis, a condition where some parts of the spine are curved; however, most recent investigations showed other concerns. In 2020, she was told to do an MRI for a more comprehensive evaluation.

My sister and I walked into the MRI centre with a request letter from the doctor. We met the doors open and the lights on, but the MRI machine was not working,” she told TheCable.

Adebisi said that although there is a building dedicated to MRI scans on the hospital premises, the machine is out of service.

“I was told that it had been faulty for a long time, so we were told to find another facility for the scan,” she said.

Then I was informed that the only place in Ibadan with a functional MRI machine was at a private diagnostic centre.”

Adebisi said she visited the centre but couldn’t afford the scan as she had already spent a lot on the other tests.

“The scan had to be put on hold for about three years. With the help of my family, the money was eventually raised, and I went back to the centre to find out that the price had ballooned,” Adebisi lamented.

“I spent almost N200,000 ($187) on the test. It was really painful because the delay went on for too long, and I had to live with the pain through the years.

Every time I went for my appointments at UCH, I used to see different patients with their MRI results from private labs or even some from other states.

“People would make jokes about why they have to spend that much money on MRIs when they don’t even have enough for food.”

Emmanuel Abiodun sustained an injury in 2018. He had an x-ray done as he was feeling some discomfort in his right knee.

His X-ray result pointed to an abnormality in the patella region, which needed an MRI for confirmation.

“It happened at the time the machine was down. I was forced to start using other forms of treatment like exercise and therapy to reduce pain,” he said.

“An MRI scan is an important clinical procedure, and it would have been nice for me to get it done then to know what the problem was and if surgery was needed.”

THE NEED FOR URGENT SOLUTIONS

“From my experiences as a medical student and now a doctor, I’ll say we are making progress,” says Babatunde Odunsi, a medical doctor at Lagos State University Teaching Hospital.

“Modern techniques of treatment and diagnosis have improved; but on the accessibility front, we are several steps behind.”

Odunsi said the lack of essential imaging or diagnostic aid in teaching hospitals points to poor management policy and governance.

“These are not exceptional or specialised tools; they are basic,” he said.

He noted that public-private partnerships increase accessibility and efficiency. He added that he expects such arrangements to work out at UCH and other teaching hospitals.

“We do not have local expertise. We do not have standard schools that train people to use these machines,” Ogbole, who is also an African Oxford (AfOx ) visiting fellow at Oxford University, said.

He told TheCable that most people who can afford to buy an MRI machine in Nigeria use it primarily for business.

“This is replicated across most African countries. Also, the few people who are trained to operate these machines have been compelled to relocate,” the professor said.

He, however, said plans are ongoing for the UCH to get a new MRI machine.

“There is an arrangement with the federal government to get the hospital a new MRI. Possibly in the next couple of years, we will get a 1.5 MRI,” Ogbole noted.

“It’s not in my purview to determine the cost of an MRI, as the new machine we are expecting is a form of public-private partnership and will also provide the technical capacity.”

Ogbole, who is a part of the leadership at the Consortium for the Advancement of MRI Education and Research in Africa (CAMERA), also recommended the use of a smaller, low-field MRI, which is easier to operate and maintain. He lamented the exodus of medical personnel from the country and the inadequate power supply in Nigeria. The low-field MRI, he said, is compliant with the uniqueness of Nigeria’s challenges and can also be used in rural communities.

“It looks like an ultrasound machine and can be used to scan a patient by the bedside in the Intensive Care Unit. The low-field MRI requires very little energy as it is battery-powered and cost-efficient,” he said.

Ogbole noted that while the downside of the machine is its low image quality, there is, however, diagnostic information that can be used for emergencies and to plan treatment.

“Investigations carried out in rural areas can be transmitted to any doctor anywhere in the world. We can begin to present these kinds of MRIs to peripheral hospitals for early diagnosis of conditions like stroke and trauma,” he added.

“A coolant is required. To keep it at a stable temperature, you need uninterrupted electricity. With the power situation in Nigeria, three or four generators are needed as they have to be switched on all year round. It also requires a lot of human capacity and trained personnel. Having the quality of trained people to manage MRIs in our environment is a challenge.”

He said biomedical engineers and the experts who operate the machines are in short supply.

Ogbole said the International Society of Magnetic Resonance in Medicine aims to democratise MRI, especially in Africa.

“Some of them are outdated already. Some of the machines that we have now are more than 20 years old,” he said.

Ogbole said the MRI machine in the UCH has reached its “logical end of life”.

THE DOCTOR’S FLIGHT AND THE PATIENT’S PLIGHT

Olushina Ajidahun, a resident doctor, said the healthcare situation in Nigeria is on the verge of collapse. He attributed this to a lack of adequate healthcare facilities and a poor reward and welfare system for health workers.

“It is embarrassing for one of the biggest government hospitals in Nigeria not to have a functional MRI. It should not be taken with levity. For example, you know that an MRI has a lot of applications and it is one of the modalities of medical investigation,” Ajidahun said.

“It hinders the doctors and the managing team as the level of their diagnosis is affected. I think that is very bad.

“It will lead to delay and inaccurate diagnosis because, without MRI, the doctors can only make guesses and could be treating the wrong ailment. This, in turn, impedes treatment planning and monitoring of patient’s progress and may lead to untimely deaths.

“Some procedures can only be done with assistance from an MRI. The machine can guide some surgical procedures, but when the machine is not functioning, then we cannot have those surgeries.”

Ajidahun said this results in patients consulting quacks because they don’t have money to do private investigations.

He noted that it is also going to impact the training of resident doctors because it is the only way they get professional experience. He advised the government to prioritise healthcare funding and rework the entire remuneration of healthcare workers.

“It would lead to a reduction of internally generated revenue in the hospital because patients have to be referred to private facilities. If you have a functioning MRI in a hospital, that’s more money for the government at subsidised rates for the government. That’s more IGR for the country,” he said.

“I think if this is done, you’ll notice that many healthcare workers would stay and try to help things get better. One MRI machine can save thousands of lives.”

Doctour is one of the healthcare professionals who left the country for greener pasture

Doctour, who goes by the pseudonym “Wakawaka Doctor” on X, formerly Twitter, told TheCable that he left Nigeria to practise elsewhere because he was overworked and underpaid.

“At the time, we’d see an average of 100–200 patients per day, and the salary was less than $100/ month,” he said.

“It’s disappointing that a big institution, a premier one like UCH, lacks access to a functioning MRI.”

“Neuroimaging in conjunction with physical examination is a valuable resource, and MRI is one of the biggest imaging techniques.”

For patients, the lack of access to MRI scans could result in delayed treatment and potentially worsened health outcomes. It may also lead to increased financial burden as patients may need to seek alternative, often more expensive, diagnostic methods.

A Multi-disciplinary approach to complex medical issues is definitely very important; by the grace of God I have never needed the type of care that necessitates experts from across disciplines to attend to me so I answered the question from a privileged perspective, granted.

That’s if those people actually exist in this country anymore. Apart from paediatrics, obstetrics and maybe cardiology, the level of specialisation is basically zero because all of them have ESCAPED. I saw somebody on tiktok lamenting that his sister can’t get treatment for a head injury because nobody in his vicinity knows how to read an MRI scan. And that was in Abuja for crying out loud!

I did not mention the calibre of staff or wealth of experience found in either institution because half of those guys pivot between both in order to supplement their income and build their portfolios.

As stated my concern is the abhorrent state of federal medical facilities across the country; even UNTH which along with UCH ought to be the gold standard for medicine and research at least in west Africa is shaping up to be a total disgrace. The last time I accompanied somebody there they wouldn’t even allow me to ease myself in the washroom without interrogating me like a criminal, is it supposed to be normal?

I think it’s short-sighted of you to dismiss the importance of good facilities in medical care; what exactly can even the best surgeon do in the absence of proper equipment? Of what use is cleaning and sterilising when clouds of dirt and dust billow in the immediate surroundings?

The risks of a cesarean delivery
A cesarean delivery is becoming a common type of delivery worldwide, but it’s still a major surgery that carries risks for both mother and child. Natural childbirth is still the best option for the lowest risk of complications.

The risks of a cesarean delivery include:
Bleeding

Blood clots

Infection

injury to the child during surgery

Longer recovery time compared with vaginal birth

Surgical injury to other organs

Breathing problems for the child, especially if performed before 39 weeks of pregnancy

Increased risks for future pregnancies

You and your doctor will discuss your birthing options prior to your due date. Your doctor will also be able to determine if you or your baby are showing any signs of complications that would need a cesarean delivery.

How to prepare for a cesarean delivery
If you and your doctor agree that cesarean delivery is the preferred option for delivery, your doctor will give you full instructions about what you can do to lower your risk of problems and have an itch-free cesarean delivery.

As with all pregnancies, prenatal appointments will have to do with a lot of checkups. This will include blood tests and other examinations to know your health for the likelihood of a cesarean delivery.

Your doctor will make sure to record your blood type in case you need a blood transfusion at some stage in the surgery. Blood transfusions are rarely needed during cesarean delivery, but your doctor will be prepared and ready for any complications.

Even if you are not preparing to have a cesarean delivery, you should always prepare for the unforeseen. At prenatal appointments with your doctor, talk about your risk factors for a cesarean delivery and what you can do to lower them.

How a cesarean delivery is performed
Prior to the surgery, your stomach will be cleaned and you’ll be prepared to receive intravenous (IV) fluids into your arm. This allows doctors to administer fluids and any type of medicine you may need. You will also have a catheter added to keep your bladder empty during the surgery.

Epidural: common anaesthesia for both cesarean deliveries and vaginal, which is injected into your lower back outside the sac of the spinal cord

Spinal block: anaesthesia that’s injected directly into the sac that surrounds your spinal cord, as a result numbing the lower part of your body

General anaesthesia: anaesthesia that puts you into a painless sleep, and is typically reserved for emergencies.

When you have been accurately medicated and numbed, your doctor will make an incision just above the pubic hairline. This is usually horizontal across the pelvis. In an emergency, the incision may be vertical.

Your doctor will first take care of your baby by clearing their nose and mouth of fluids and clamping and cutting the umbilical cord. Your baby will then be handed over to a hospital attendant and they will ensure that your baby is breathing very well and prepare your baby to be put into your arms.

If you’re certain you do not want any more children and have signed the consent, the doctor can tie your tubes (a tubal ligation) at the same time.

Your doctor will fix your uterus with dissolving stitches and close your abdominal incision with sutures.

Following up after a cesarean delivery
At the end of your cesarean delivery, you and your new baby will stay in the hospital for about three days. Directly after surgery, you will be on an IV. This allows for adjusted levels of painkillers to be taken into your bloodstream while the anaesthesia wears off.

Your doctor will persuade you to get up and walk around. This can assist in preventing blood clots and constipation. A nurse or doctor can teach you how to position your baby for breastfeeding so there’s no extra pain from the cesarean delivery incision area.

Your doctor will give you recommendations for home care after the surgery, but you should expect to:

Go gently and rest, especially for the first few weeks

Use correct posture to support your abdomen

Drink plenty of fluids to replace those lost in the course of your cesarean delivery

Seek assistance if you experience symptoms of postpartum depression, such as severe mood

Swings or overwhelming fatigue

Avoid sex for four to six weeks

Take pain medications as needed


Contact your doctor if you experience any of these symptoms:

Breast pain that comes with a fever

Foul-smelling vaginal discharge or bleeding with large clots

Pain when urinating

Signs of infection — for instance, fever over 100 °F, redness, swelling or discharge from the incision site.

Where do i start..
I am my parents only child, i honestly have lived a very controlled and careful life. I first had intercourse at the age of 27 with a lady i was planning to marry but it didn't click because of certain issues. I wont consider myself a good Christian as i only remember God when i am in serious issues. But anyway, i have been careful all my life. A lot of my friends experimented with drugs, women etc, but i persisted with a clean and honest living. i graduated university at the age of 20, got a job, relatively good job, whilst still serving, so my life so far, had been comfortable. I have never struggled with anything in my life. My parents, both are teachers, would always remind of with a proverb in my place ' A single palm fruit does not get lost in the fire whilst been roasted', literally meaning, if you have a single palm fruit roasting in the fire, you have to carefully monitor it, unlike when you have a lot roasting. Figuratively, i was their only child.
Well in march 3rd on the eve of the lagos state gubernatorial election, i met a girl. I had known her from her workplace, although we did not have sexual intercourse, We somehow touched each other. Unfortunately for me, earlier that day, i had injured myself, just a pin prick on my thumb. Whilst touching this lady, there might have been bodily fluid being mixed. Immediately after this incident, when my senses have become less overwhelmed by my sexual desires, i noticed the injury. Instead of going to the hospital for prep, i simply asked God for forgiveness and forgot about it. I got to understand, that actions begot consequences.

In late march, i went on vacation to the UK, i had a lot of alcohol, i came back to work and since then, my life had been miserable, one health issue or the other.

First came, heart pain(Angina), jaw, shoulder and left had pain, i shrugged it off and started exercising, then came Neuropathy, i felt pain in my legs most especially whilst lying down, well, i increase my dosage of exercise. After all, i was young.

There was silence, my health issues were over, i thought, but then at the turn came stomach issues, i had severe diarrhea, bloating, nausea. I shrugged it off, it must be a stomach bug or something i ate whilst on vacation. I ate things that i knew would not sit right in stomach literally.

Whilst i was battling with the questions of my stomach biome, came fever, it was terrible, it wrecked me. I had never been this sick in life. I had malaria before now and knew all the symptoms that came with malaria, but this time around, it was malaria plus something i could not put my hand into. I never had headache and heart pain with malaria, but this time, i had meningitic headache, the centre of my skull was soft, i could push it in and my heart was racing and pacing as if i ran, even while sleeping. My Resting heart rate which i always monitored before now as sometimes above 100 while sleeping. I went to the hospital and after several test, they claimed i had malaria and typhoid. l started treating the malaria first but it was not getting better, i then took amoxillin, the headache seized but my symptoms doubled. I still had malaise, deep within me even though i was feeling better because the pain and headache had stopped, i still felt sick and my heart raced on.
Other symptoms speedily appeared, my lymph nodes got swollen on my groin, my neck had multiple swollen lymph nodes, my head also had swollen lymph nodes which were painful. Also i noticed i had severe oral candida, my nightmares began, i had very stupid and weird dreams.

I visited a nurse who did a full blood count and noticed that my lymphocytes were quite high, 58% of my WBC, i was also anemic and speculated that this would most likely be a viral infection. I am not very versed with virology, so first thing i concluded was maybe HIV. I knew my life had changed forever. I delayed the testing because i was afraid, i was due marriage, the pressure was coming from my parents, i also had plans of futhering my studies and relocating abroad, with this virus, these might come to naught, but instead of testing, i festered the thought, i carried this secret alone, i could not tell anybody my fears.

More symtoms then came, my tongue not just went white with candida but terrible cancre sores developed on my tongue, these sores would go and new and more numerous sores would emerge. Also i developed rashes on my laps due to anemia, i took supplements, i visited different hospital praying for a different diagnosis apart from HIV, i literally prayed, God give cancer instead of HIV, i joined various HIV groups on reddit, i spent so much of my salary in the hospital that i had to go back borrowing from my parents, but i couldnt tell them why i suddenly started borrowing.

More symptoms came, my hair started thining, i was going bald, went i comb my hair, clumps of hair just fell off, i felt terrible, every morning, i spent so much time in bed reading peoples experience with the symptoms of HIV, and guess what, i had all the symptoms even HIV Neuropathy, instead of going to work, i spent a lot of times on blogs reading people first symptoms of HIV that i was always late for work. The candida was not going any where.

More symptoms came, i developed a pain at my left back, went for various xrays and ultra sound and they found nothing, i did a kidney function test and it was okay, i did a UTI test and it came back okay, then i did an urethra swab and they found i had staph which like candida is a disease that affects people with poor immune function or immunocompromised people, also i found out, that i suddenly developed ulcer.

More symptoms, because of the heart issues, symptoms and stress of the sickness and the whole mental turbulence i found myself in, i became hypertensive, my blood pressure rose from 128/78 to 167/88, i had random headaches and eye pain, i started burping more tha 70 times in a day.

One day, i woke up after year of this mental torment, i something from a novel i had read in the past hit me, although contained in the bible, but the novel had exposed the true meaning of this to me, "THE TRUTH SHALL SET YOU FREE". On my way to the office, i bought a HIV unigold test kit, i also ordered a oraquick from Jumia,

This experience had a good aspect, as i drew me nearer to God, before now, i always thought the power to succeed depended on one's hard work, i would always quote Achebe "no god makes a man prosperous but the strength of his hands and diligence", will your hard work count if you have been bedridden by sickness, i think not.

I prayed like never before, i cried before God and begged everyday for a second chance. I would be better, i will be nicer to my parents and people around me, after all to love another person is to see the face of God. My health further worsened, my skin became sensitive to touch and i would cry out in pain if someone lightly touched my skin.

The French mathematician philosophy called Pascals wager is simple, the benefits of believing in God outweighs the so benefits of not believing in God, simply explained, let say you die and there is no God, what did you lose by keeping his commandments, maybe the luxury and things of the world, but that is little compared to the pain and anguish you would go through in the torments of hell, if God exist and you fail to believe.

John Lennox also points something rational to believing in God, his argument goes like this 'lets say an atheist losses a child, well you can only tell the atheist condolences, nothing more, but a Christian, you would remind that the child would of course be in eternal rest in the bossom of Christ. Thus Christianity is even more humane as it provides hope and faith for the loses we suffer here on earth, and for the evil that happens in the world, where real justice can be perverted, we are reminded that Christ is the true judge and we will get justice at the end of our life, thus as Christ whilst on earth, went through suffering, we might suffer, have faith that we will rise with him and get our reward, So Christianity is even more rational than atheism'.

I also reminded myself that even in the situation that i HIV that God also presents me with a second chance to make a difference by getting treated early since HIV is no longer a death sentence but a chronic illness not worse than diabetes.

I got tested, first using the UNIGOLD blood prick in the office, my heart raced preparing myself for the worst, but hoping for a second change.

It fortunately for me was non reactive to the virus, did another test with the Oraquick device, also non reactive and went to the hospital to confirm, still non reactive.

I still have some of the symptoms and hoping on God to make my healing complete. If you will, give your life to Jesus, he is a better driver than you are and he will take care of you. Just remind him of his words, keep a personal relationship with him, speak to him as you would a friend, if you sin, he will never deny or reject you. Remember, this is another chance for you as you have heard this words a thousand times, i pray that the holy spirit to would speak to you, as i have done my best, he is the great comforter and pray that this would not stand against you on that last day.

Health / How Rot In LUTH Led To Us-based Nigerian’s Death by Paraman: 8:40pm On Feb 26
The Editor-in-Chief and Chief Executive Officer of The Source magazine, Comfort Obi, in this article, narrates how Nigeria’s chaotic health system, typified by the rot in the Lagos University Teaching Hospital, Idi-Araba, Lagos, led to the death of a United States-based Nigerian, Mr Olaleye Adenibuyan

On January 24, 2024, Nigeria lost a patriotic citizen. It lost Mr Olaleye Adenibuyan. He died in circumstances that, at once, confounded, broke the heart, and put a big question mark on Nigeria’s healthcare system.

To not a few people, Adenibuyan’s death was avoidable if only the hospital where he died lived up to its assumed status of a teaching hospital.

Let me confess upfront so you don’t accuse me of being deliberately emotional.

Adenibuyan was my cousin-in-law. A fine gentleman, he was married to my cousin, Thelma. And we admired and loved both of them “die”, as young people would put it.

Theirs was a relationship built on a solid foundation; a partnership rooted in time. They loved wearing uniforms and pranced around like teenagers who just fell in love.

Adenibuyan had served his country, Nigeria, as a police officer before he relocated to the United States of America in 1989. But that relocation never stopped him from visiting his beloved country, his beloved Ondo State, and his more beloved community, Owo, two times every year.

For him, it was a ritual. His love for Nigeria was that strong. And each time he visited, he bought more local fabrics for uniforms for himself and his beloved wife Thelma. In their local fabrics, they promoted Nigeria’s culture, and fashion.

So, this year, 2024, as usual, he set out from his Dallas, Texas, USA base for Nigeria and arrived Lagos on January 14, 2024. Each time he and his wife came home, either together, or separately, they usually checked into a hotel at Ajao Estate. The Estate is close to the Lagos Airport.

For the Adenibuyans, it was convenient as it saved them from the punishing Lagos traffic (we call it go slow) to the airport for a flight to Akure, Ondo State, en route Owo.

So, on January 14, Mr Adenibuyan arrived Lagos and checked into the usual hotel. Luggage did not arrive from the US and so, he needed to buy some things from a shop opposite the hotel. That done, as he climbed up the staircase back to his room, the devil stepped in. Tragedy struck. He missed a step. And fell backwards.

As he fell, the family was told, he hit his head on the floor or wherever. The impact was grave. He lost consciousness. And was quickly rushed to a nearby hospital. I cannot confirm what attention he got there. His state was beyond what a small private hospital could handle. So he was quickly referred to the University of Lagos Teaching Hospital, LUTH.

Established in 1961, LUTH is a tertiary hospital affiliated to the University of Lagos College of Medicine. It is a 761-bed hospital established to be a centre of medical excellence. To its credit are some of Nigeria’s best brains in medicine. Many of its products are those “making waves” worldwide. They were trained there. It used to be Nigeria’s pride. As were the University College Hospital, UCH, affiliated to the University of Ibadan, and the Obafemi Awolowo University Teaching Hospital, OAUTH, affiliated to the Obafemi Awolowo University, former University of Ife. And some more.

I don’t know about others, but LUTH has lost its status as a centre of medical excellence. It is now a shadow of itself; a shame to Nigeria. It has deteriorated. With Adenibuyan admitted there, we experienced, firsthand, the shadow LUTH has become. And our hearts broke.

The injury Adenibuyan sustained to the head needed urgent attention. It was a medical emergency. So, he was admitted to the intensive care unit – private wing, no less. Meaning the attention was expected to be top-notch.

When one pays millions of naira, even as the naira has lost its value, the least one would expect would be first-class attention. But not here. There was nothing special. Patients were kept in what I choose to call “an open mini ward”. No privacy. No screen. When the question of some privacy was raised, the answer was: “It is because there is no general monitor.”

Once Thelma heard of her husband’s situation, she began to make arrangements to come home. She works in one of the biggest and best government-owned hospitals in Texas where she has risen to the position of director. So, once she was briefed on the prognosis, she knew she had to rush back to Nigeria. Her mission was to take her husband back with her to the USA once he was stable enough to fly.

Meanwhile, from the US, before she was able to secure a seat on a plane, she and the family rallied around to pay every kobo required, every kobo, directly and indirectly, demanded, officially or unofficially. No expense was spared.

But what did the family see at LUTH?

LUTH had no equipment. Nothing. After the millions of naira deposited, one still had to pay, separately, for soap and gloves. For a scan to determine the extent of damage to the head, Adenibuyan was taken to a private facility outside LUTH. Why? LUTH said its own scan machine was not in “a working condition.” A teaching hospital? The scan showed a lot of blood in the skull. Nothing was done.

A couple of days later, LUTH declared triumphantly that the “bleeding has stopped”. The question we, as laymen, asked was: What about the blood already accumulated there? Our elementary understanding was that the blood “has caked there!” If true, we were nervous about the implication.

More surprises were afoot.

On January 17, three days after he was admitted, LUTH said Adenibuyan needed an intracranial pressure monitoring machine. But this teaching hospital does not have the machine. When needed, it was explained to us, it is rented from outside. Cost: N400,000. The family paid. But the machine was not delivered until January 19th. And when it was delivered, it was left by the corner of Adenibuyan’s bed for days, unused.

Perhaps, it was a coincidence, but the ICP monitoring machine was used only on the day Thelma arrived (24th) and began to ask questions. This was 10 days after he was referred to LUTH, and perhaps, 10 days after it should have been used.

Thelma arrived in Nigeria at about 9.40 am on Delta Airlines and went from the airport to LUTH to see her husband. She waited for about three hours before she was allowed after which she incessantly requested to speak with his medical team. She wanted to know why the ICP had not been put in place as was revealed to her by Lekan, her stepson, who was in Nigeria for a short vacation, and her husband’s younger brother, Deji. She wondered why the machine was just lying down there. When one of the doctors finally arrived, he tried to explain.

But given Thelma’s background, and where she came from, the explanation made no sense to her. She hinted so in many ways, but was, at once very disciplined and too distraught to argue. But finally, she was told another doctor who would do that was being expected.

The doctor, an unassuming guy, competent, calm, and collected finally arrived. We were sitting in the ICU waiting room when he walked past. Instinctively, and I guess, from his carriage, I knew he was the one, and I told Thelma so. She sent a message across that she would want to speak with him.

Over an hour later, the doctor came out from the ICU and asked for Thelma. We followed him. And Thelma had a lot of questions and complaints. He listened, and said he had just returned to the country the previous day, and was seeing Adenibuyan for the first time, but quickly added “he is being attended to by a good team.” He explained to us where he thought he should, and apologised where he thought he should. For example, he agreed with Thelma that it was not right to intubate her husband without informing the family. He apologised it was wrong not to have carried the family along every step of the way. And then, calmly, he told us what the situation was, and the way forward.

He said Adenibuyan required an urgent surgery to release the pressure on the brain. He disclosed that the pressure was 61, far beyond the normal 15. This was what Thelma and Lekan consistently, subtly, suggested and appealed for a surgery to release the pressure to the brain. It would entail the removal of a part of the skull bone to allow the brain to swell and then, compress later to normal size. This should have been done, at most, three days after the unfortunate incident.

Anyway, better late than never, we consoled ourselves.

The time for the surgery was set for 4.00 pm. But again, a problem.

LUTH does not have a drill. The family was told “There is only one place to rent it. Cost N200,000. No problem. This was on a Thursday. The surgery was meant to be done immediately. But the rental place said, “The drill is not available until Friday afternoon”. Another vendor was frantically sought. He agreed to N180,000 and promised to deliver it against the 4.00 pm surgery time. Great. Our spirit lifted some.

But another problem.

Unbelievably, LUTH does not have more than two functional surgery rooms. So, there is usually a queue. Adenibuyan had to wait. One doctor, obviously frustrated by the situation told us: “Today two are functional. Tomorrow, Friday, only one will be available.”

So, I asked why: He told us: “We have 22, but there is no manpower. Doctors, nurses, and technicians must have left. If the 22 are open, there will be nobody to man them. Nobody. So, why keep them open?” We were appalled. Our hearts sank. But we held onto hope.

So, either as a result of the queue, or the unavailability, yet, of the drill, the surgery was shifted from 4.00 pm to 8.00 pm. I left and told Thelma I would be back by7.00 pm. But just before5.00 pm when Thelma went in to see her husband again, his health had taken a nosedive. Even then, the man who hadn’t opened his eyes for 10 days, opened them once he heard his wife’s voice. She held his hands tight. “Baby, you know why I came. I came for you. We are going back together. I will put you on a flight. We go back together. Your treatment will be taken care of in the US. And, you will be perfect. We’ll be fine, you and I.” The three doctors Thelma met, and she told them the same thing. “I am going back with my husband. That’s my mission. To take him back to the US with me.”

That was not to be. While Thelma held his hands, and CPR was being performed on him, he gave up. He died in his wife’s arms.

Since Adenibuyan’s passing, too many questions remain unanswered about our country’s healthcare system. Take LUTH for instance.

It is not that there are still no qualified medical personnel, even with the exodus, but here is the problem. There is no medical equipment. The medical personnel are just managing, barely managing. Or, how does one explain that a teaching Hospital, LUTH, no less, does not have a functional scan machine, does not have ICP monitoring machine, or the equipment for drill?

It is the shame of a country. As I said earlier, it is not the problem of the medical personnel. I admit that the work ethic of a number of them is zero. Compared to what we see in some other climes, they need a re-orientation. There is no sense of urgency. At times there is no empathy. But I also admit that their work environment is a major problem. It is not inspiring. I admit that their welfare is a major problem. It is depressing. I admit that knowing what to do, and not having the equipment to do it is frustrating. One of the doctors who spoke to us out of frustration said: “You are talking about the equipment. Where is the manpower? Because of our situation, most of us have left. A number of those remaining are on the verge of leaving.” When I asked if he was on his way out too, he gave a knowing smile. I helplessly shook my head.

Since Adenibuyan’s death, regrets have been our food. Many “ifs”. What if he hadn’t been referred to LUTH? Perhaps he would still have been with us. What if LUTH had used the ICP machine as at, and when due? Perhaps, he would still have been with us. What if the drill was used at the time it should have been used, perhaps he would still have been here with us. What if some sense of urgency had been exhibited, perhaps, he would still have been here with us.

The Federal Government shamelessly laments what negative effect the ‘’Japa’’ syndrome has had on Nigeria’s healthcare system. It shamelessly tells us that 42,000 nurses have left Nigeria in the past three years. Why not? How has the Federal Government treated them? What have you given them to work with? Now, shamelessly, it is putting obstacles here and there to stop nurses from leaving. Why? My response is in one word: Shame.

Isn’t it a shame that the Nigerian government, from state to Federal Government which throws money around as if it is going out of circulation, cannot boast of one good government hospital except Lagos State.? I am reliably informed that Lagos State University Teaching Hospital, LASUTH, affiliated with the Lagos State-owned Lagos State University, LASU, is very well equipped by the Lagos State Government. In our doubts at LUTH, one woman called us aside and asked: “Why did you come to LUTH? Why did you not take him to LASUTH? This type of injury is better handled there.”

We spend tons and tons of money, billions of dollars, trillions of naira, on frivolities, on things we can do without. How does one explain that $6.2m was spent, allegedly, without authorization, on foreign election observers when LUTH has no medical equipment? Of what use was the presence of the foreign observers to the masses? Did their presence stop us from rigging, from snatching ballot boxes, from doctoring results? Nigeria spent this money when LUTH had no medical equipment, not even a functional scan machine. How does one explain that the sum of N1bn was recently requested to enable a committee to fix workers’ salaries? Yet, our premier hospitals are empty. Can you imagine what that obscene request could have done for LUTH?

But back to Thelma. We don’t know how to handle her, or what to tell her. She is distraught. Disoriented, almost. Her mission to take her husband home to their “second country”, the US, blew up in her face. “Oh, your husband loved you to death. He waited for you to come back, to see you before he passed on. He even opened his eyes for the first time in 10 days once you arrived”, Thelma is told in a bid to console her. Where do all those leave her?

All she knows is that the Nigerian healthcare system failed her. Her mission to take her husband back to the US with her failed. She was, at a point, making inquiries for an air ambulance to evacuate him to the US. That failed. Ironically, what worked was taking him back to Owo in a body bag! Sad!!

Mr Adenibuyan, as your beloved wife fondly called you, may your soul rest in peace. May you find peace in the fact that you are finally, finally back to your cherished Owo.

Where do i start..
I am my parents only child, i honestly have lived a very controlled and careful life. I first had intercourse at the age of 27 with a lady i was planning to marry but it didn't click because of certain issues. I wont consider myself a good Christian as i only remember God when i am in serious issues. But anyway, i have been careful all my life. A lot of my friends experimented with drugs, women etc, but i persisted with a clean and honest living. i graduated university at the age of 20, got a job, relatively good job, whilst still serving, so my life so far, had been comfortable. I have never struggled with anything in my life. My parents, both are teachers, would always remind of with a proverb in my place ' A single palm fruit does not get lost in the fire whilst been roasted', literally meaning, if you have a single palm fruit roasting in the fire, you have to carefully monitor it, unlike when you have a lot roasting. Figuratively, i was their only child.
Well in march 3rd on the eve of the lagos state gubernatorial election, i met a girl. I had known her from her workplace, although we did not have sexual intercourse, We somehow touched each other. Unfortunately for me, earlier that day, i had injured myself, just a pin prick on my thumb. Whilst touching this lady, there might have been bodily fluid being mixed. Immediately after this incident, when my senses have become less overwhelmed by my sexual desires, i noticed the injury. Instead of going to the hospital for prep, i simply asked God for forgiveness and forgot about it. I got to understand, that actions begot consequences.

In late march, i went on vacation to the UK, i had a lot of alcohol, i came back to work and since then, my life had been miserable, one health issue or the other.

First came, heart pain(Angina), jaw, shoulder and left had pain, i shrugged it off and started exercising, then came Neuropathy, i felt pain in my legs most especially whilst lying down, well, i increase my dosage of exercise. After all, i was young.

There was silence, my health issues were over, i thought, but then at the turn came stomach issues, i had severe diarrhea, bloating, nausea. I shrugged it off, it must be a stomach bug or something i ate whilst on vacation. I ate things that i knew would not sit right in stomach literally.

Whilst i was battling with the questions of my stomach biome, came fever, it was terrible, it wrecked me. I had never been this sick in life. I had malaria before now and knew all the symptoms that came with malaria, but this time around, it was malaria plus something i could not put my hand into. I never had headache and heart pain with malaria, but this time, i had meningitic headache, the centre of my skull was soft, i could push it in and my heart was racing and pacing as if i ran, even while sleeping. My Resting heart rate which i always monitored before now as sometimes above 100 while sleeping. I went to the hospital and after several test, they claimed i had malaria and typhoid. l started treating the malaria first but it was not getting better, i then took amoxillin, the headache seized but my symptoms doubled. I still had malaise, deep within me even though i was feeling better because the pain and headache had stopped, i still felt sick and my heart raced on.
Other symptoms speedily appeared, my lymph nodes got swollen on my groin, my neck had multiple swollen lymph nodes, my head also had swollen lymph nodes which were painful. Also i noticed i had severe oral candida, my nightmares began, i had very stupid and weird dreams.

I visited a nurse who did a full blood count and noticed that my lymphocytes were quite high, 58% of my WBC, i was also anemic and speculated that this would most likely be a viral infection. I am not very versed with virology, so first thing i concluded was maybe HIV. I knew my life had changed forever. I delayed the testing because i was afraid, i was due marriage, the pressure was coming from my parents, i also had plans of futhering my studies and relocating abroad, with this virus, these might come to naught, but instead of testing, i festered the thought, i carried this secret alone, i could not tell anybody my fears.

More symtoms then came, my tongue not just went white with candida but terrible cancre sores developed on my tongue, these sores would go and new and more numerous sores would emerge. Also i developed rashes on my laps due to anemia, i took supplements, i visited different hospital praying for a different diagnosis apart from HIV, i literally prayed, God give cancer instead of HIV, i joined various HIV groups on reddit, i spent so much of my salary in the hospital that i had to go back borrowing from my parents, but i couldnt tell them why i suddenly started borrowing.

More symptoms came, my hair started thining, i was going bald, went i comb my hair, clumps of hair just fell off, i felt terrible, every morning, i spent so much time in bed reading peoples experience with the symptoms of HIV, and guess what, i had all the symptoms even HIV Neuropathy, instead of going to work, i spent a lot of times on blogs reading people first symptoms of HIV that i was always late for work. The candida was not going any where.

More symptoms came, i developed a pain at my left back, went for various xrays and ultra sound and they found nothing, i did a kidney function test and it was okay, i did a UTI test and it came back okay, then i did an urethra swab and they found i had staph which like candida is a disease that affects people with poor immune function or immunocompromised people, also i found out, that i suddenly developed ulcer.

More symptoms, because of the heart issues, symptoms and stress of the sickness and the whole mental turbulence i found myself in, i became hypertensive, my blood pressure rose from 128/78 to 167/88, i had random headaches and eye pain, i started burping more tha 70 times in a day.

One day, i woke up after year of this mental torment, i something from a novel i had read in the past hit me, although contained in the bible, but the novel had exposed the true meaning of this to me, "THE TRUTH SHALL SET YOU FREE". On my way to the office, i bought a HIV unigold test kit, i also ordered a oraquick from Jumia,

This experience had a good aspect, as i drew me nearer to God, before now, i always thought the power to succeed depended on one's hard work, i would always quote Achebe "no god makes a man prosperous but the strength of his hands and diligence", will your hard work count if you have been bedridden by sickness, i think not.

I prayed like never before, i cried before God and begged everyday for a second chance. I would be better, i will be nicer to my parents and people around me, after all to love another person is to see the face of God. My health further worsened, my skin became sensitive to touch and i would cry out in pain if someone lightly touched my skin.

The French mathematician philosophy called Pascals wager is simple, the benefits of believing in God outweighs the so benefits of not believing in God, simply explained, let say you die and there is no God, what did you lose by keeping his commandments, maybe the luxury and things of the world, but that is little compared to the pain and anguish you would go through in the torments of hell, if God exist and you fail to believe.

John Lennox also points something rational to believing in God, his argument goes like this 'lets say an atheist losses a child, well you can only tell the atheist condolences, nothing more, but a Christian, you would remind that the child would of course be in eternal rest in the bossom of Christ. Thus Christianity is even more humane as it provides hope and faith for the loses we suffer here on earth, and for the evil that happens in the world, where real justice can be perverted, we are reminded that Christ is the true judge and we will get justice at the end of our life, thus as Christ whilst on earth, went through suffering, we might suffer, have faith that we will rise with him and get our reward, So Christianity is even more rational than atheism'.

I also reminded myself that even in the situation that i HIV that God also presents me with a second chance to make a difference by getting treated early since HIV is no longer a death sentence but a chronic illness not worse than diabetes.

I got tested, first using the UNIGOLD blood prick in the office, my heart raced preparing myself for the worst, but hoping for a second change.

It fortunately for me was non reactive to the virus, did another test with the Oraquick device, also non reactive and went to the hospital to confirm, still non reactive.

I still have some of the symptoms and hoping on God to make my healing complete. If you will, give your life to Jesus, he is a better driver than you are and he will take care of you. Just remind him of his words, keep a personal relationship with him, speak to him as you would a friend, if you sin, he will never deny or reject you. Remember, this is another chance for you as you have heard this words a thousand times, i pray that the holy spirit to would speak to you, as i have done my best, he is the great comforter and pray that this would not stand against you on that last day.

On July 18, 2023 this Youtube star Annabelle Ham died at the age of 22😢. She was young, healthy and attended a bachelorette party so I started researching why she died at such a young age.

Then I discovered that she died from an Epileptic event known as SUDEP (Sudden Unexpected Death in Epilepsy).

Doctors say patients with uncontrolled seizures are at greatest risk for SUDEP.

So today I would like us to talk about EPILEPSY.

Epilepsy is a disorder in which the nerve cell activity in the brain is disturbed, causing seizures.

Epilepsy is also known as: seizure disorder.

Epilepsy may occur as a result of a genetic disorder or an acquired brain injury, such as a trauma or stroke.

In all you do brother's and sister's, please avoid head injury. I know someone who had serious head inquiry almost 20 years ago, he just started having seizures since 4 years now.

Epilepsy Causes
In most cases, epilepsies are caused by genetic factors, traumatic brain injury (TBI), developmental brain abnormalities, infection, stroke, brain tumors etc.

Growing up in Nigeria, I noticed that once someone is having a seizure people would put spoon in their mouth and everyone would be shouting "pour am water" lol.

So how do you help someone epileptic?

1. DON'T put anything in the mouth of anyone having a seizure
2. Don't try to hold them down
3. Don't spray or pour them water
4. Place a soft cloth or pillow under their heads to minimise injuries.
5. Lay them on their left side to avoid spit or vomit entering their lungs
6. Remove tight clothing or belts
7. Take away any dangerous objects around them, eg wires, sharps etc
8. Time the episode, if it lasts longer than 5 minutes, take them to a hospital
9. The person has another seizure soon after the first one, take them to the hospital.

Epilepsy may occur as a result of a genetic disorder or an acquired brain injury, such as a trauma or stroke.

In all you do brother's and sister's, please avoid head injury. I know someone who had serious head inquiry almost 20 years ago, he just started having seizures since 4 years now.

Epilepsy Causes
In most cases, epilepsies are caused by genetic factors, traumatic brain injury (TBI), developmental brain abnormalities, infection, stroke, brain tumors etc.

Growing up in Nigeria, I noticed that once someone is having a seizure people would put spoon in their mouth and everyone would be shouting "pour am water" lol.

So how do you help someone epileptic?

1. DON'T put anything in the mouth of anyone having a seizure
2. Don't try to hold them down
3. Don't spray or pour them water
4. Place a soft cloth or pillow under their heads to minimise injuries.
5. Lay them on their left side to avoid spit or vomit entering their lungs
6. Remove tight clothing or belts
7. Take away any dangerous objects around them, eg wires, sharps etc
8. Time the episode, if it lasts longer than 5 minutes, take them to a hospital
9. The person has another seizure soon after the first one, take them to the hospital.

No, you aren't pushing in this one. You're pulling as always, but this time you pull with one hand on the head, and another hand at the base. Classically, Kegels are used by women to help make their nether regions feel tighter. However, men also have the same pelvic floor (PC) muscles that contract during orgasm and halt the flow of urine so Kegels can be useful for men as well. To perform a Kegel, you flex to contract the muscle. Begin with 20 to 30 repetitions of this contraction throughout the day and work your way up to a higher number. Making this love muscle stronger helps maximize the blood contained in the penis during erection as well as maintain erection quality, which will be important as your penis grows in size.

3. The Majestic Jelq

Jelq is supposedly where the term “jerk,” as in “jerking off,” originated. It's claimed to be an ancient enlargement method. Essentially the jelq is a “milking” motion of the semi-erect and lubricated penis. Taking one hand in a gentle yet firm grip, make an OK sign with the thumb and forefinger at the base of the penis, then slowly drag the hand up the shaft to the head of the penis. Repeat with the opposite hand. Be gentle and start with a low number of repetitions before working your way up. Jelqing helps to increase the girth of the penis and adds a little bit of length too.

An alternative to the basic stretch involves pulling the penis outward to stretch it and then rotating it clockwise. Hold this position for 30 seconds, and then repeat the stretch while rotating it counterclockwise. Perform this sequence five more times, for a total of six repetitions. It is recommended to engage in this regimen once per day, as it may contribute to increasing penis girth over time.

This is a fairly advanced penis enlargement practice and not for the feint of heart. You simply attach a weight of some type to the penis, letting gravity do all of the stretchings for you. In time your penis should increase in length. This particular method was supposedly used by the Chinese and achieved the desired results in most cases.

However, this method can cause great injury if it's not done correctly. For that reason alone it is highly recommended that you perform the previous exercises mentioned in this guide before ever considering the weighted stretching method.
