What do we call this, is it a mistake or they actually know what they wrote and report?

It was Months back that CNN made such mistake to name Lagos State as Capital of Nigeria instead of Abuja, now SkyNews just reported same while reporting Ebola outbreak and 45 death in 3days.

Is it all a mistake or they have reasons of such little mistake?
