You worry too much. If you swear a correction of name affidavit and they say it's wrong, you can go back and do the right one it won't even take up to an hour.

Calm down brother. The issue is, everything will depend on the decision of your clearance officer. just swear an affidavit first as a backup to the Conflicting names, then when you appear to be cleared physically, follow the instructions of your clearance officer. To start with, an affidavit Should be an awaiting succour, go swear one.
