Dr. Julz: I did tell you earlier before we went on a break that I’ll be joined by a dermatologist; that’s the skin specialist, she’s here finally. Thankfully, the traffic did not swallow her up for me. We have Dr. Ehiagi Anaba, welcome Ma.

Dr. Anaba: Thank you and thank you for having me on the show.

Dr. Julz: Alright perfect, so we’ve been talking about unhealthy skincare practices. And they are a lot of them, the next one I’ll be talking about is, in short I would say 80% of us are guilty of this one. It is pressing your phone against your cheek, I think most people that have touch screen, they now put it away from their face because of that notion, not because they are aware, what will you have to say about that?

Dr. Anaba: Well, there are many things that can cause acne which is also known as pimples. Most people just know acne as pimples, now what you’re talking about is one of the causes. It’s called the mechanical aspect of acne causation. It’s not just the pressing of phone on your cheek, people who have this bandana around their head, people who plays the violin. Basically what happens is, you have these things pressed against the skin, which blocks the pores. Most of the time the things that actually cause acne is the fact that the pore itself is blocked. So the cells themselves cannot come out of those pores, then you have bacteria acting on what is trapped inside the pores and then you the break-out of acne, so not just your phone especially our teenagers, they are perpetually on the phone… (cut in)

Dr. Julz: There are so many packages out there now saying it is easy, call the entire Nigeria all for free.

Dr. Anaba: They don’t even hear you anymore, all they are doing is; you are talking to them somebody is engrossed in their phone, so persistently having this on your face are things that have pressure on your face…(cut in)

Dr. Julz: And they radiate heat as well.

Dr. Anaba: Yes, they radiate heat. Then something else, some people could be allergic actually to the materials that these things are made of, but they don’t even realize it. They just see themselves breaking out in all kinds of rashes and I have to say here that it’s not all rashes on the face that are acne.

Dr. Julz: Good, go ahead…

Dr. Anaba: Everybody thinks everything on the face is acne, there are so many things that can cause rashes, so what you need to do is, see your dermatologist. And I have to say something, now that takes us to something of using wrong skin care products…(cut in)

Dr. Julz: That is number 5.

Dr. Anaba: Everybody I see in the clinic, they’ve used Funbact A… (cut in)

Dr. Julz: Oh, my goodness…laughs… oh my God! Sometimes I get educated by them about the products, I’ll say, can I have a look? You know one of the things I talked about is our habit of people playing doctor, you just go to somebody, it is so convenient for Nigerians to meet somebody that is not qualified to ask for medical treatment

Dr. Anaba: Do you know that sometimes I’m in the pharmacy, I see a woman coming with her child who has rashes and she’s asking the pharmacist; what should I apply? And I’m wondering are you OK? The least you can do is at least take this child to a pediatrician. Pediatrician will tell you, ok, you need a dermatologist or you need to do a test….. A chemist…. and by the time they are done with their Funbact A, they’ll come to me and I’m wondering where do you want me to start from?

Dr. Julz: Now please emphasize on how dangerous that thing is because apart from a parent taking a child to… you know, you are talking about a pharmacy, what about, I don’t even know what to say. Some people hawk drugs, they will go there and you just see them and they ask anybody to give them medication. How dangerous is this?

Dr. Anaba: These are very dangerous practices. What I do not understand, funny enough is it cut across all our social strata

Dr. Julz: Exactly!

Dr. Anaba: Sometimes I see people in the clinic and actually ask; how well educated are you? And they’ll tell me, and I’ll say then you did these things? Because in this country just don’t think about their health. We are more interested in “I’m carrying my Prada, my Gucci, I’m wearing my six inches, what about your body, your health? We are not interested. Now those creams I mentioned, Funbact A, Endicin, TrippleM and they are brothers and sisters. The main thing in them is steroid. And these people actually notice, because some tells me; they notice that when they are using these creams they actually gets lighter, so not only do they apply these creams to where they have their lesion, they need it for their whole body…(cut in)

Dr. Julz: Because of how they’ll look. But what I have to mention now is that we are not condemning these products, it’s just the abuse… (cut in)

Dr. Anaba: The abuse of the product and they use these products wrongfully.

Dr. Julz: Exactly!

Dr. Anaba: There is a place for steroid use in dermatology and there is a time frame. Because when they place themselves on these steroid creams, what it does is; (1) It makes the skin thin, the skin actually becomes thinner, yes it does makes them lighter, also it makes the effect of the sun on that skin even worse.

Dr. Julz: Exactly, that is why they have the black burns or the blue-black….(cut in)

Dr. Anaba: Because these people do not realize that they’ve taken out the ability of the skin to protect themselves from sunshine and they do not use sunscreen. If you are doing something wrong to your skin, you should be using something else to correct it. They won’t use sunscreen; they will not moisturize that same skin, so the skin ends up looking not too fantastic.

Dr. Julz: I think at this point I should just advice anybody that is using any cream from a tube, as their regular daily regimen you should just stop it. Because most of those creams were designed to be used for treatment. Am I correct?

Dr. Anaba: They are designed for treatment, for a specific time and for specific rashes, not for everything and then also, when you keep using those creams, what they actually do is; they hide the real rash that you have, that when you do see a doctor, it’s difficult for them to figure out what exactly you have. For me when the patients come, I have to tell them you need to stop this cream, so that the rash that you actually had would come out so that I can know what to treat, because what I tell them is, now what you are showing me is that somebody visited you, you know when you are not in the house somebody come visiting you, leave a calling card so you know, Mr or Mrs A came visiting me, what the person wanted you really don’t know, that’s what you are showing me here. I don’t not know what was on your skin before, but I know that there was something there, because of what you have done to the skin.

Dr. Julz: What is the most dangerous diagnoses that you’ve ever made, that a patient have covered up, knowingly or unknowingly with some over the counter medication?

Dr. Anaba: now, most times of all the skin care products they use, steroids are the most dangerous. The problem with steroid is, not only is the work on the skin, it also does things inside your body. Ok, it does things inside your body and makes you unable to withstand stress so to speak; so if you fall ill or other things happened to you, your body on its own is unable to help you because you have rendered it helpless. I’ve had a patient, she came to us. The reason why she came to us was that she just had diarrhea, she had diarrhea and because of that she had lost too much fluid that the kidneys shutdown. But before we re-hydrated her she got better and then I realized that this lady wasn’t getting better the way other people would get better, so we tend to ask more questions; she turned out she had used steroid creams for a long time and because of that she had shutdown what is called her hypo-adrenal axis. That is just the part of your body that help you to withstand stress and because she was with us, she was not using that her cream anymore so she had double jeopardy, I have to sit her down to say what brought you here we’ve sorted it out since, within the first 3 days that you came here, what brought you here we’ve sorted it out. Why you are remaining here is what you’ve been doing to your body yourself for years. And I try to educate her, but another problem we have in this country is when you tell people things, the truth; you don’t see them anymore. Most times we lose this patient to follow up, you start looking for them, you can’t find them and we all know that in this country, most places you really can’t trace the address. Even when we call our social workers to go look for these people it’s difficult to find them.

Dr. Julz: My Goodness! If you are just tuning in, you’ve tuned on to Health Matters with Dr. Jues and in the studio with me is a dermatologist Dr. Anaba. And we are talking about the 10 unhealthy healthcare practices. Skincare practices that we have in our society today. We talked about not concentrating on below the neck; when you take care of yourself apply your cream, that’s the first one. The second one we talked about is keeping sunscreen; you must use your sunscreen. Third one we talked about you picking your pimple, your acne, stop it. The fourth one is pressing your phone on your cheek for a long time. People that use your cap for a longtime… Even wigs because it’s irritation and you don’t clean it…. and if you use it constantly on your forehead of the side of your face, it’s irritating and then you called it a term that it was mechanical acne. And we then mentioned using wrong skin care products like the steroid cream. Now, you might say steroid, and people might like say, what is steroid? Now, let us mention not the name of the cream, not the product, but those contents that people should look out for. The steroid you should look out for when you are getting this cream, so that you don’t fall victim to this….(cuts in)

Dr. Anaba: they don’t fall victim I can assure you, they did it on purpose…(cuts in)

Dr. Julz: If they know that these are the health implications, they will not do it, I believe.

Dr. Anaba: They know it. I see these people all the time, they will walk into the clinic and they’ll say; I don’t want to be dark, I want to be light in complexion

Dr. Julz: Nobody will come and meet you and say, look I’m having problem with urinating, my kidneys have failed and I know it’s this cream that is causing it and I still want to use it. If they are aware, if people are enlightened they would not use it.
