No, you do not.

I just registered on the portal now and I have sent in my application. It worked without hassles and the form was quite easy to fill. I think the only information one should have on hand are NYSC certificate number and your height. The rest can be filled offhand without consulting anything. I got a mail informing me of the successful submission of my application. Now we wait for them to get back to us.

It said same for me too o but just once. The captcha reloaded by itself and the second trial went through. Maybe you should try now as the site seems to be working fine.
