It's an American faster way/shorter form of 'How're you doing?' It's has nothing to do with with being educated or not. It rolls off the tongue better, that's all. I watch American shows, and white people say it more than blacks... blacks have another slang altogether - 'what'sup?'

Brits have their own version... 'Wotcha' which is an economic way of saying 'what are you doing?'
