Firstly genitals means sex organs or a collection of it. it really matters a lot that it should be in its rightful condition even before getting married.
Your reproductive system can't be functional if your genitals are malfunctioning, I mean not performing the way it ought to, maybe because of sexual transmitted diseases.
Another aspect is hereditary factors, like someone having a low sperm count not because of STD but because it runs on the family blood.
