Background
Nigeria has one of the highest maternal and child mortality rates worldwide, with the rates much higher in underserved rural populations. The major reasons for these poor health outcomes are low and inequitable coverage with high impact cost-effective interventions, which is largely due to the very weak health system, especially at primary health care level and low demand for health services. The National Primary Health Care Development Agency (NPHCDA) has been providing leadership in revitalization of PHC in its quest towards accelerating progress towards Universal Health Coverage (UHC).It is globally acknowledged that UHC is infeasible without strengthening the community component of PHC. Hitherto, this level of care had been characterized by verticalization, fragmentation and limited impact. Consequently, the NPHCDA has spearheaded the harmonization of all community-based programs into the Community Health Influencers, Promoters and Services (CHIPS) Program, and all the community-based workers into the CHIPS Personnel. These personnel comprise CHIPS Agents and the Community Engagement Focal Persons (CEFPs). All the different community-based health worker training curricula have been integrated into the CHIPS Training Curriculum.The CHIPS Agents are mostly female, drawn from the communities they reside in, selected in collaboration with members of their communities, through the Ward Development Committees. They are expected to have a minimum of primary school qualification. A minimum of 10 CHIPS Agents are to be selected in each ward. They are expected to work in assigned households, creating demand for primary health care services, with a focus on maternal and child health, tracking pregnant women and infants and providing basic treatment of children with cough, diarrhea and fever. They are also expected to identify and refer pregnant women and under-fives who are ill and found to have danger signs to primary health facilities. They also link families and communities to primary health care services. Two male CEFPs are selected per ward; their main responsibilities are supporting the CHIPS Agents with data summarization and transmission, community mobilization and engaging with the men to address barriers to maternal and child health services utilization.One of the key principles of the CHIPS Program is volunteerism; consequently, the CHIPS Personnel will not be paid any salary. There is therefore the need to have means of livelihood if the Program is to be sustainable, a female empowerment component has been added to the Program to address this need. The empowerment component will focus on training the CHIPS Agents on entrepreneurship, income generation and agency. Following the training, the CHIPS Personnel will be supported to form and register cooperatives and subsequently linked to sources of grants/loans.NPHCDA is working with key stakeholders to articulate the empowerment component of the CHIPS Program. One of the key recommendations of the stakeholders is the development of the CHIPS empowerment strategy of the Program and exploring opportunities for leveraging/partnering with other social safety net programs for the implementation of this component of the Program.
