INCREASE IN HEART BEAT RATE:
Within two minutes of inhaling tobacco, the carbon-monoxide present in the tobacco smoke increases the blood pressure and the heart beat rate respectively.

Haemoglobin is a protein in the blood that transports oxygen around the body by binding with oxygen.  However, once we smoke, the carbon-monoxide in the smoke has a greater affinity for binding to Haemoglobin than oxygen (over 200 times greater than oxygen).  The carbon-monoxide displaces oxygen and bind with haemoglobin; as a result of this the blood carries less oxygen.  This means that the heart of a smoker does extra work by pumping blood at an increased rate to be able to transport enough oxygen to the brain, heart, muscle and other organs.

Secondly, the complex compound which carbon-monoxide forms with haemoglobin is a stable compound (carboxyhaemoglobin).  It is not reversible like oxyhaeoglobin which it forms with oxygen, because of this; the percentage of free haemoglobin in the blood is reduced continually once they bind to carbon monoxide as you smoke.

Note: oxygen is essential for cellular respiration and our brain cells can only live for a few minutes without oxygen.
