In simplified terms, the exhaust note is the end production of combustion. The fuel-air mixture that is compressed within the cylinders is ignited. This cause an explosion, which forces the piston down, rotating the crank. On the exhaust stroke, the burnt gases leave the combustion chamber via the exhaust valves. It is this explosion that we hear. Of course, the silencers absorb most of the noise, before these gases exit the tailpipe. So what? On another note - what is the essence of your thread?
