This survey is intended to measure fraud supervision as part of an organisation’s risk management process. We would like accurate information as such, stay anonymous by leaving out your name or organisation.

The following can be classified as fraudulent activities:

• Conflict of Interest: Colleague or upper management has a personal interest which influences the exercise of his or her official duties. (e.g. Inappropriate vendor or customer relations, bribery).
• Embezzlement: The use of property for personal use. (e.g. Bookkeeping errors, misapplication of funds, and mishandling of cash).
• Ghost Employees: An individual recorded within the payroll system who does not work for the organisation.
• Falsification of Contracts or Records: Altering or falsifying any part of a document for the purpose of gaining an advantage.
• Theft: Stealing, use or misuse of employer’s assets without permission to do so.
