The individual is most likely not your brother but YOU. Also, I highly doubt you're (your brother is) 20. I may be wrong but here's the thing about false information, especially when reporting to a doctor. Lying and leaving out important details about your illness leads to wrong diagnosis. And guess what wrong diagnosis leads to? Wrong medication. Medication that will not solve the problem but also lead to far worse issues.

As some intelligent folks on here have mentioned, go see a health care professional. Seek one out at a top medical facility in your area or elsewhere. Stay away, I repeat, stay away from those herbal lairs. These are low life crooks trying to get their filthy paws on your hard earned money. Go to teaching hospital for treatment. It's kidney related but treatable. Nephritis something something.... It's very common among kids and teenagers.

He is actually right you know.. swelling of eyes, arms and legs results from loss of kidney function.

I took a physicians course on Kidney disease and there I got to know kidney disease has stages from stage 1 through 4 or 5 thereabout when it becomes Kidney failure. Foamy urine and protein in the urine are early symptoms of chronic kidney disease (CKD) and early treatment most times slows the progression and in some cases dialysis and kidney transplant is required.
I know a guy who suffered these exact symptoms described by the poster but sadly didn't make it. Guess he couldn't keep up with the treatment but it's quite sad.
