If it's a renewal you don't need them.
For the letter, a sample is pasted on the wall at the servicom desk. You can use that or simply address your letter to:
The Comptroller General
Nigeria Immigration Service

Having gained from the thread, I guess I should share my experience for others to benefit.
Mine was a renewal; my passport will expire in April 2020. I wanted the 10 year validity passport; this is only available in 64 pages and is N70,000. I filled the form online, chose Ikoyi office and paid in Zenith bank. I tried to pay with my card but it took long for the OTP to arrive on several occasions; hence I decided to go to the bank. I paid N70,500.

Required documents
Old passport booklet (They will sight and return to you)
2 passport photos on white background
Copy of NIN slip
Letter of application for renewal (handwritten)
Copies of old passport data page
Printed online form
Acknowledgement slip
Payment slip
2 forms you'd be given at the servicom desk.

For renewal, you DO NOT need:
-guarantor
-state of origin confirmation
-birth certificate
-court affidavit

After the filling the forms, I checked and rechecked all information for correctness before proceeding to pay. Once you request to pay, you'll see a lot of payment channels, I chose Sanaa pay and got a print out which I took to the bank and paid.
I returned and checked my profile and noticed a capturing date had been assigned: 31st Dec. 2019.
Dday, I arrived the Ikoyi office at about 9am with all my documents and proceeded to the servicom office. I was given two new forms to photocopy and fill. Photocopies cost N50 per page; N100 for 3 pages.
Returned and filled out the forms; the officer brought out a file jacket and enclosed all the documents, itemized the stages and sent me off to the next desk at servicom. Here, the file was reviewed again, the officer signed did some registration in a book and sent me off to the long room. One of the officers at servicom took the files (there were two others guys DIYing like me; just three of us  at the time and our files were different from the ones being assisted by NIS officials), she handed it to the guys at the long room (verification I guess) and ensured they attended to the files and even proceeded with them to the capture room for enrolment. She enrolled us whilst we joined the queue for biometrics. The queue moved quickly and I was done in no time. They printed a form with an enrolment I.D and other details; collection date printed on the form was 3rd January 2020. I returned to servicom with my file and the form, the officer at servicom took the file and advised me to check in two weeks because they didn't have booklets for the 10 year at the time. Can't remember precisely but I think I left the NIS office at about 10.30am.

I returned to passport office on Friday, 17th January, went with the enrolment form I was given on the day of capturing. I went to the servicom desk first, the lady took my form and typed something in the computer and told me it was ready. She ticked on the form and told me go to the collection room for pick up. I got there and it was crowded. I was told to drop the form in a basket and wait for the officers to pick up and go retrieve the passport. 15 minutes from when I dropped my slip an officer came out and packed all the slips in the basket. From then, I waited for 1 hour 30 mins; the officers came out severally but I still didn't hear my name. I got uncomfortable and as I stood up to go complain, I saw the lady that had attended to me and confirmed my passport was ready; she was walking towards the collection room, I ran after her and told her I was still waiting, she told me to rewrite my name on a piece of paper and give her. She went into the office with it and reemerged with my passport. I joined the queue for activation; this took another 30 minute wait. The entire collection process took 2 hours as I arrived at 11.50am and left at about 1.50pm.
Overall, I think it's a worthwhile experience except for how long collection took; my passport expires in 2030 grin, so mission accomplished...everyone should DIY!!!

My CA quest began in December 2017 with Ielts, I knew about the EE process before then but was uninterested initially. I took up the challenge when I realised that I didn't want to ever regret not taking on the opportunity at the right time.

IELTS
I wrote the exam a whooping 4 times which actually felt like 40 times. The first was with BC, I wasn't prepared but I aced all except Listening. For the want of nearer dates, I went on to MOD for the second attempt which in my opinion was a wrong decision and all sections were fine except Writing. This happened again the third time and I dashed back to BC and aced all. I remember someone telling me after my third ielts attempt that maybe God did not want me to go, well I just gave a blank look, nodded and thought to myself that if I wasn't going to make it to Canada as a PR, it wouldn't be ielts to stop me. After my fourth attempt, I wasnt sure of my performance, so I filled Nova Scotia forms in preparation for the site to open; also wrote down the required TEF scores to boost my CRS. I wasnt giving up. Don't let that stumbling block deter you. Keep pushing and your success is sure.
Practice, as they say, makes perfect and if you know what you want, you'd never stop till you get it. If ielts is still in your way to achieving this dream, make up your mind and never give up on it.

ECA
I have a Masters degree with a U.K university which made WES a ready option. I registered on WES website and faxed in my certificate. I requested transcripts from my school via email which I didn't have to pay for. ECA was ready in record time without hitches.

MEDICALS
Had my medicals at Children's Practice. I arrived the hospital before 8am, doors opened at 8am and we were attended to according to a list of how we placed our bookings. We were given tellers to pay at the bank; I paid and returned for my tests. I completed the entire procedure at about 10am.

PCC
Inspector Moshood to the rescue for this. I was done with it in 30 minutes and I paid N4000.00.

POF
I booked treasury bills for my POF and got a reference letter from my bank in cic format. I got a request format from page one, drafted it, signed and sent to my account officer. I merged the treasury bills certificate and bank reference letter for POF.

EMPLOYMENT DETAILS
I claimed a continuous 3 years from one NOC and got a reference letter from my employer in the required format. I merged the letter with my offer letter, promotion letters, pay slips and I.D.

EDUCATION
I merged ielts TRF, WES ECA, MBA certificate and first degree certificate.

DIGITAL PHOTO
I see a lot of people snapping and uploading passport photos in the format for POST-PPR. I did the same and later realised it wasn't necessary since the hard copies are only required POST-PPR. Please there's no need to get hard copy passport photo until you get PPR; you can use an old one but just make sure it in the required size.

I submitted my application on 27th July 2018, then the wait began!
I requested GCMS notes directly from ATIP in September (at $5) which came in October, exactly one month after (someone living in Canada has to apply for you and you'd need to sign a consent form authorising CIC to release your information to them). The notes revealed that everything looked good except that file had been transferred to AVO.
My application clocked 6 months on Jan. 27th and I emailed AVO and received the generic 'file is in queue for review' email response. I also emailed some MPs including Ralph and Ahmed Hussein; received generic responses from them too and no further details.

On Thursday 21st February, ADRs started flooding in for July AORs.
I received mine on 22nd, it was for the same set of documents; 5 in number (we tagged them 'famous five'):
*Pay slips from employer for the period of assessment
Period of assessment was really confusing so I sent pay slips from when I started the role till date.(4years plus)
*Bank statements showing salary from employer
I didn't know if this was meant to be the same as the pay slips but I decided to send 6 months statement from present day; I highlighted salary payments as requested.
*Updated proof of funds
I had rebooked treasury bills upon expiration so I went to the bank for a reprint of the certificate (just so it would have present date), I merged this with a reference letter in CIC format which I also got from the bank on that day. Reference letter stated my salary account balance, average six months balance and treasury bills.
*Schedule A Background Declaration form (IMM 5669) bearing an original signature for you and your spouse. Please ensure there are no gaps in time.
Filled this, printed, signed and scanned.
*Additional Family Info IMM5406: A separate and newly completed Additional Family Information form (IMM5406) bearing original signatures for you and your spouse.
Also filled, printed, signed and scanned.

I submitted the ADR February 25th and we all thought we'd get PPR in a few days but AVO wasn't done yet; the wait continued. Then in the week of April 8th 2019, the rain started. Daily, we counted over 10 PPRs mostly May, June and July AORs.

Then it happened!!!
I received the golden mail!!!
I have been so overwhelmed.

Here's my timeline:
ITA:  July 25th 2018
AOR: July 27th 2018
MEP: Aug 20th 2018
IP1: Aug 20th 2018
Requested GCMS: Sep 26 2018; received Oct 29th 2018 (ATIP)
ADR: Feb 22nd 2019
ADR submission: Feb 25th 2019
IP2: Apr 10th 2019
PPR: Apr 15th 2019
Passport submitted: Apr 17th 2019
Application Approved: Apr 30th 2019
COPR received: May 8th 2019

So why AVO? I read in different places that straightforward applications were treated in Ottawa which is the fastest VO and complicated ones were sent to local visa offices that would better understand the culture and expectations of the area. Well if having previous visa refusals from U.S and France are complicated, then I guess that criteria is correct. In my opinion though, every application is different and it might just be a thing of timing and luck to determine where applications are sent to. Most importantly, the only thing consistent about IRCC is inconsistency. On the other hand, the AVO trend has kinda changed so don't be downcast if your application is sent there, just trust God that all will end well.

I kinda concur with you on the bold part above about MOD.
I wrote IELTS 3 times, though I got the result that I needed on the 2nd attempt. My writing test with MOD which was my last attempt returned a band 6.5- guess what, it was my best "writing" of all the 3 attempts grin grin whilst the first two attempts with British Council delivered band 7 on each occasion...and to think this MOD writing is a queer reoccurring matter leaves a bad taste. sad

I pray ITA falls on you soon bro.

NB: My MOD test returned: LRWS- 8, 8, 6.5, 7 (I can understand the frustration, the disappointment and the thought of going back into the trenches again for many here...wow) I pray for those writing and re-writing the IELTS that everything-the weather, the difficulty level of the test, the state of your body and mind, time usage etc- will work in your favour to make it your last attempt IJN!



It will end in praise IJN!

Bless the Lord with me people... I am free from IELTS. Actually told myself it would be the last whether good or bad.
I had planned not to check my result till evening because my previous three attempts had me crying all day in the office and I end up having a bad day, but anxiety got the best of me.
Test day I got confused during the map section which left me unprepared when the next section began (Matilda n co). I counted 9 questions I wasn't sure of and I gave up and even started planning and gathering docs for Nova Scotia PNP.

1st attempt LRWS: 7.5,7.5,7.5,8 BC
2nd attempt LRWS: 8,7.5,6.5,7 MOD
3rd attempt LRWS: 8,8,6.5,8 MOD
4th attempt LRWS: 9,7.5,7,7.5 BC

Only God could have done it.
I have been a silent reader on this thread and I've learnt so much; I joined cochtrane's webinar for Listening after my first attempt. God bless you sir. I have gotten most of the requirements ready except PCC and medicals.
If you are still on the ielts journey, don't give up. I know it's easy for me to say now, but it's the truth. Pick up from the last try and push forward.
Besides, I think MOD writing is stricter, I think I wrote better essays in my second and third attempts. I ran back to BC since I did well at first attempt even being not so prepared. It's just my opinion though.
Thanks to all the beautiful contributors here; you guys are the best. Plus I enjoy d drama too.
