Betrothed means engaged. Simply, the verse is saying once you are engaged to a lady, you can have sex with her before marriage if she is matured enough and the man feels like. Afterwards, the guy must marry the lady.

BTW the one who said we should forget abt the old testament is wrong. Jesus said he didn't come to tell us to forget abt the old testament, but to amend it. So we still follow some laws in the old testament.
