Why can't we have permanently attached trailers like this, instead of putting containers on a flat bed that is always falling off and crushing people.

Op, you say "Zero Survivors", eye witness says "No casualties'!

I have said it here and keep saying it that trucks like these have no business moving in the daytime with other motorists, at least not till this safety issue is permanently addressed!

We just don't value human lives in Nigeria... If we did, this should still be happening over a decade after!

It simple means that via design, the bolded wasn't put into consideration. Hence, by function it is failing due to the bolded. A problem known is half solved already.

Like I said, it's very had for it to fall off because of its weight and balance.

Yeah it might dangle left and right at some point due to road. But it takes extreme pressure to get it off the trailer. Or when whatever is in the container is not well placed to even the weight distribution.

Then you transfer the contents of the containers into the American styled permanently attached trailers, this is not rocket science. You cannot keep transporting containers on flat beds through Nigerian pot hole filled roads without accidents like this becoming constant.
