I currently work with a multinational company here in Nigeria, I have a son and my wife to take care of.

My wife is a registered nurse.

I want her to travel abroad for further studies, I have some savings already. And I eat. #1.5 m+ monthly.

Please can you suggest best countries she can go for studies and also with prospects of getting permanent residency soonest.

Hahahhaha. You are joking. It will be so shocking for you to know that most private trauma hospitals in Nigeria are far more equipped than government hospitals.
Infact government hospitals send their patients to private hospital facilities for some tests like Xray, ct scan and MRI.

Over the years I have been in control of my finances but my finances are controlling me this year and it is really telling on my health.
I have been borrowing from loan apps and paying back but I'm finding it difficult to save. My rent will expire in two weeks time and I don't have a dime on me. The little I was able to save I had to spend it on feeding the family. I don't want to continue borrowing because it makes me not to have peace of mind and always at the edge.
