This is absolutely wrong if it is affidavit for attestation of birth you are making. There is no way your mum's name should come into it as the said affidavit should be about you all through.

Go to any Federal High Court around you and look for those typist type-writers around the court using old fashioned type-writer. Tell them you need Affidavit for Attestion of Birth.

To the best of my knowledge, most Affidavit goes this way,


Firstly .....The title

The body of the affidavit would start like:

I, Ade Ademola, female, of No 3 Nairaland avenue, Lagos, No Mans Land, Nigeria, a Muslim (or Christian if you are), a true Citizen of the Federal Republic of Nigeria, do hearby make an oath, and state as follows:


1. That I am the despondent herein
2. That I was born in Nura Hospital, Lagos on 18th November, 1995
3. That my correct date of birth is 18th November, 1995......
4......

Don't use exactly my format but this are similar things yours should have. Those typist are in best position to tweak this and get you a correct Affidavit that would be accepted. And remember, you will pay them for the draft plus payment for them to get it stamped and registered in the Federal High Court.

Alternatively, you can try to get it done online, which will have digital verifiable code. But you have to be sure the informations you input and the type to put in to avoid such rejection again.

I did this and it worked but NPC rejected my application saying there was somethings required in the affidavit that I didn't add . Do you have any idea

what exactly are you modifying in NPC?
or is it just the birth certificate/attestation you just wish to get?

And can you give little information on what you have in your affidavit? (the exact wording while removing your personal data on it...
this will give an idea of what could actually be missing in the affidavit)

Find a white background (wall, curtain, block , ceiling etc) and try and take the photo with light on.

Try and stay away from the light rays directing towards you. Also try and do it close to windows so that natural light will help counterbalance the light in the room.

Do it during day time. if not working , try do it at night around 7pm - 9pm with the room fully illuminated (and lieing on bed while trying to get the capture...the bedsheet will be your background colour )
