Anyways, I always go to Google and search any random address of any country/state/city/town to use when necessary.

Also, always make sure you use the mail you Used to sign up for it to register for any survey.

As for phone number, Google is your friend too, pray the survey doesn't require you to verify the phone number.

Check your IP city, state and zip code on whoer.net if not available do some digging on Google to find the city

Go to maps.google.com

Search the zip code, city and state e.g

Zoom into the location and you see box like images click on any you will get address that align with your IP location.
