Also what I mine going to fill for where's they asked for CONTACT NAME AND TELEPHONE on delivery receipt? See the picture of delivery receipt I attached below .

Take every other documents to the embassy.

Please remember to photocopy your covid-19 vaccination card. If you have cac cert, photocopy it and go with the original for sighting.
