I don't think anyone on that particular date have gotten, i did my Biometrics same day at Ikeja...nothing yet..probably tommorow they would drop collection mail straight

Yes they do. If you are refused, you will get a refusal mail.
In this case, i think his visa has been granted already.

I got a decision mail yesterday and another mail saying my travel documents are ready for collection, both mails are from TLS. but someone is making me scared right now that the UK embassy is supposed to send me a message saying my documents are ready else it's refusal, please can I speak with anyone who has gotten his visa recently?

If it's a refusal, you will be sent a refusal mail.

to those still waiting to get a decision mail, i want to say their is no need to really be worried as long as you did the right and and submitted the right documents, mine took around 45 working days back then in September 2020, sent a paid email asked for it to be escalated but their was no change in application status, mailed school to complain, school got in touch with them (note different uni with different mode of operation) and after some days i got a mail from school that my application had been granted the next day i got a decision mail and after another 5 working days i got the collection mail. what am trying to say in essence put too much pressure on yourself you will surely get it.
To those who have gotten their visas, please make sure you start online lectures its very important, safe trip when you guys land and dont forget to call the NI office few days when you arrive so they can send your application form to you so you can

Although, I received a phone call from my bank(Access), to confirm if I sent a statement to Uk embassy, then the following day which was Tuesday, I received a non-straightforward mail

11,700 on the day of your Biometrics for assisted service. They would upload it for you.

I'm in a bit of a dire situation. I chose self service in my application for UKVI for upload of documents. My appointment is for Friday. I am unable to log in to the UKVI portal in other to upload my supporting documents. I have tried my email as my username and my password it still tells me invalid username or password. Please what can I do to sort out this issue between now and friday

Go back to your UKVI application portal, click on provide Biometrics and Documents, it will link you back to the TLS portal. I had the same issue logging in directly through the TLS website.

It was refused.
You appealed the decision.
The appeal was dismissed.
So your initial refusal stands.

This is only one application, one case and hence one refusal, I don't see how it can be considered to be two separate refusals.

In your UK visa form, indicate it as only one refusal from whichever country that was.

I'm not hiding any detail. You probably don't understand the scenario. However did you use the option of assisted service of 11,700 for uploading omiited document?

I think you should say you have been refused only once, as it's one application that was appealed.
This is my opinion only - in your shoes I would use the date of whatever appeal decision you received in September, but I think it really doesnt matter because whatever case/reference number your matter had assigned will most logically be a continuation of or refer back to the original July decision, so you have declared your immigration history, which is what they want.

I have an Enquiry please, i had a refusal in July and I appealed, the appeal was refused in September, when filling my UK application, am I to say have been refused once and explain and should I use the latest date of refusal?

The question is why are you writing a cover letter? did you miss out on a document, fund calculation/analysis, visa refusals? If so, then yes, you can use the service to upload your cover letter.
The ones i have written in the past were addressed with;

Please I forgot to change the name of the document when uploading, it's supposed to be fidelity bank, but I left it as cert3, would be a problem?

Ordinarily once a positive decision is made on your application, it automatically locks your visa for printing at the visa production section,afterwhich they dispatch. But currently they are not printing and dispatching approved visas because of the lockdown.

On the white print out, there is AVATS number if you actually know what it looks like. It is written there as Bank receipt number, directly below your passport number.

I don't think your right, cos the bank receipt number starts with 92
