If we use your submission that Hitler ruled France, then rulership would be too lose a term. I am very sure the French wont list Hitler as one of its ruler. A military occupation translates into rulership when resistance has been totally/subtotally crushed and rulership is established post-occupation.

The Americans/Russians occupied Germany after their victory in WW2. Both it’ll take more than a strong notion to say Americans/Russians ruled Germany.

Britain became a protectorate of Roman Empire due to the fact that it was written that they were unable to wage war with a stronger Power... So much less they'll be regarded as colonies tho... In Nigeria, we had the Southern and Northern protectorate.
Using ur story,what would we say that the Southern kingdoms which we clearly know abt fell to British but were still tagged as protectorate? Would we say that the British conquest of Nigeria is false because South and North were called protectorates

They became a colony after their amalgamation and full inculcation into the British Empire which happened after 1914. Prior to that both protectorate were self-governed with interest from the British government. It might seem pedantic but they are different. As a colony between 1914 to 1960, they were wholly under British rule and governance.

Its like saying Hitler ruled France because he occupied France during the 2nd World War. This is something similar

As at now, the Oba doesn't rule any place that is outside of edo state... He is now ceremonial, but there are historical reports that talks abt Benin ruling Lagos indirectly

Apparently, there would be back and forth on this matter. However, like I said, I believe the Benin Empire had a great influence on the creation of Lagos via King Ashipa; as Lagos was seen as a potential war camp. Tributes were definitely paid to the Oba of Benin since Lagos was seen as a protectorate rather than a colony. And that is often obtained in empire of olds. Most nations prefer peaceful negotiations than go to war against a very powerful empire. Through this point though, Lagos was independently a Yoruba nation.

I used an example of Britain (Anglo Saxons) being a protectorate of the Roman Empire during Emperor Claudius time. The British preferred not to wage war against the stronger Roman. But even the staunchest pro-Roman supporter would tell you Britain was a Roman colony.

Please, your recount of the history is dubious. Yes the Benin empire must have attacked and maybe successfully infiltrated Lagos. But claiming Lagos as a vassal of Benin is like saying Britain is a vassal of the Roman Empire. The Oba of Benin was not and still isnt the ruler of Lagos

My baptism into the curse we experience in the Nigerian health sector started from my very first posting which was emergency pediatrics in OTCHEW, UCH. At some point some of we house officers were spending almost a quarter of our salaries on those indigent children in the spirit of selflessness. We just couldn't afford to see the babies die. If you don't have money for your baby's IV drugs we move on to the next baby. It's pathetic.
Imagine. I am currently undergoing residency in a teaching hospital (Paediatrics) and what is currently happening is ludicrous. The ARD has gone on strike for 2 months now while the MDCAN has joined yesterday. Still the state government doesnt care. We registrars just selflessly report to the hospital without pay to prevent avoidable deaths. Tell me, how would anyone still blame me for the problems of health in the country? Havent we tried enough. I would be sleeping in the hospital tonight to run the Children Emergency Room with an hungry stomach and no pay because I selflessly dont want a child with Type I DM to pack up while I am gone. Still I am the villain. My brother/sister, hope you’ve started writing PLAB/USMLE?

Many folks don't know there is what we call rural posting which is mandatory for any Nigerian trained medical doctor. Let them keep speaking in the nonsense. When you're in a rural setting where nobody can afford an x-ray these people will query you for not ordering an MRI. It's better to ignore sometimes.

He’s actually right. Few ACTs are OTC as well as Artesunate, Mefloquine, Chloroquine etc....However, there are some ACTs and even some brands of Artesunate and Mefloquine that are controlled drugs. Although Arthemeter and Lumefantrine is a Prescription only drug. I am however pretty sure Nigerian patent sellers and pharmacies dispense without prescription.

Thanks dear... It is actually discouraged... BT when u av someone in a rural setting who might not be able get to a working health facility, u might just be forced to prescribe... And like is usually adviced for most OTCs, if symptoms persist, pls present to the nearest hospital
Sincerely, my NYSC was a funny experience. I had to buy RDT myself from Lagos when going back. Ordinary neonatal suction for resuscitating new born we no get. Instead of directing our anger to the right source, we abuse the same people that make do with the little provided.

All of us go soon run comot this country sha. Na herbalist go remain. Lol

Maybe he doesn't know there is poverty in the land and not everyone can afford ordinary malaria test.
I have been to different angle of the world and sometimes MD don't conduct some test if the symptoms you shows its so obvious unless as a patient you actually want to confirm before prescription. Doctor visit alone in US is $100 not testing oh. Now if you want to carry out some test you know how much that is.

Thanks jare. I just feel its time to start educating the masses on what they see as gross inadequacies by doctors. I see a lot of posts like this on Twitter where grievances are misdirected.

An average Nigerian comes to the clinic with features of stroke but still doesnt want to do a CT or MRI because he cant afford it. Still i would write a CT Scan only despite needing both and use my crude medical knowledge to determine if its Ischemic or Hemorraghic. Something an European doctor wont imagine is possible.

Then they watch Greys anatomy and shout at how the doctor requested for MRI. How many Nigerians can afford MRI? How many have health insurance that covers such?

This is why a lot of people got to HIV 2 because they met irresponsible doctors that treated them for malaria when a simple test would have shown their actual ailment.
Well, na you sabi. If you think my job as a doctor is assumption not making diagnosis based on your clinical presentation, i guess you should visit the lab technician and treat yourselves.

I served in a rural area, a village with no lab within miles. I am sure many children who I have helped treat or prevent Severe malaria either presenting in coma or seizures would appreciate my decisiveness and fast diagnosis. I am pretty certain their parents wont call me irresponsible or call for my license.

So if u out of ignorance see all those, its your cup of tea. All love.

I'm not a doctor , so I can't even begin to argue this but everytime I've gone to a hospital even for something as common as a headache , the doctors has insisted on testing .

The only place I get malaria prescriptions without testing are chemist shops or independent pharmacies .

And there are host of diseases that have malaria-like symptoms .
HIV for one can often come with malaria-like symptoms two weeks after infection .

Hepatitis a also has malaria-like symptoms at the initial stage.

I don't know about you , but a doctor who just recommends a prescription simply because the disease is supposedly an endemic is highly irresponsible and should lose his medical license.

But then again , you say this only happens in Nigeria .
A country where patient confidentiality isn't even respected .

You say you’ve attended hospitals where tests where done before you were treated. That is not abnormal and that is even recommended especially in private centres where as a doctor you make money for your institution. Working in a teaching hospital, I have learnt to understand my patient and learn empathy towards their pocket. The NCDC and Min of Health encourages prompt RDT before treatment of malaria but allows treatment commenced when you identify the classical symptoms: Fever, headache, chills and rigor. As a patient, you are also allowed to self commence as much as you contact your doctor on the 3rd of use if no changes.

I guess you want to educate me on differentials of malaria...lmao. I spent 8 months reading Robbins and Cortrans reading on all pathologic conditions known to man. I am sure I would name 40 other conditions before you get to 5. So rest.

Educate yourself on the paragraph about losing one’s license. It’s ludicrous. As a medical student in Africa, you are drilled on making clinical diagnosis without the help of investigations. Its our strength and why we are admired all over the world. If you think MDCN would take my license for doing something i am paid to do, you are the one needing a CT Scan then.

Your last paragraph shows your mentality. Nigerian doctors are sought after across the world. Bring cases of anyone denying his/her patients confidentiality without being punished. We are cherished every where due to our lack of dependence on investigations and ridiculed in Nigeria for it. Shame

Not advisable in a professional settings to prescribe without lab test, if you must do that, you must tell them that if symptoms persist after two days, the patient should see a doctor but team two seem to haves changed their script.
Lol....a lot i wish i could explain but it’ll probably take days. When it comes to malaria in Nigeria, you can prescribe without lab test. All you need to do is give an appointment for 3 days later. Any patient who isnt well still would come back before then. Most i know wont come back again... cheesy

A lot of conditions present as malaria...Dengue fever one of the rarest. However, with experience you pick up complications that points to other differentials. Last last, as a doctor you have to be sensible though. If i suspect you have malaria, i place you on antimalarials but still send for an RDT cos though I am guided by the law, the patient’s life too is important.

So that doctor who treated your brother would/could have avoided the complications if he had done a test while treating him for malaria.

My point tho is: it is within his legal capacity to start treating without a test.

It is not wrong. The Min of Health guidelines allows us to treat malaria when you see symptoms strongly suggestive of malaria due to its endemicity and our propensity to reduce the need for investigations in Nigeria; as a result of poverty.

Malaria is endemic in Nigeria. Peculiar symptoms are likely to be malaria than any ailment. As a doctor, especially in rural settings, you prescribe antimalarials if your patient presents with classic symptoms even without tests (especially high risk patients: Pregnant women and Under 5 children). Although the mass introduction of RDTs by the Fed Govt has made test easier, even though that itself isn’t reliable.

My point is: you can prescribe antimalarials without investigation in Nigeria.
