When you indicate, "References/Referees are available on/upon request", how then are you to present the contacts of your references? Is it by getting a letter from each of them recommending you or by simply writing it down for them (the HRs) on a piece of paper? *** I know the writing it down kiniko might sound funny but, please explain.

It's safe to include your referees in your CV.
Recommendation is mostly after you are in the process of being given offer letter or confirmation.
