Thanksgiving has always been for me a time to be thankful for all the blessings in my life. But how do we live thanksgiving? I found the answer to that question in an article written by Wilferd A. Peterson called, The Art of Thanksgiving. In this article he talks about acts of thanksliving. To quote from this piece, Mr Peterson writes: "The art of thanksgiving is thanksliving. A pilonidal sinus is a small pit that occurs at the bottom of the tailbone or coccyx that can become infected and filled with pus. Pilonidal means a ‘nest of hairs’. Sinus tract is a abnormal narrow tunnel in the body which goes between a focus of infection in deeper tissues to the skin surface. This channel discharges pus occasionally on the skin, Pilonidal tract is a sinus tract which contains hairs. It appears under the skin between the buttocks (the natal cleft) a short distance above the back passage (anus). It goes in a vertical direction between the buttocks. However, in some rare cases, pilonidal sinus can appear in other parts of the body. What causes a pilonidal sinus? There are no clear reasons for the incidence of pilonidal sinus, Some theories say that people were born with this minor abnormality between the buttocks. Therefore, it can be inherited. In this condition, the hairs grow into the skin rather than outwards. Another theory says that pilonidal sinus occurs due to the development of skin dimples in the skin between the buttocks. Presumably, they develop as a result of local pressure which damages the hair follicles. Growing hair in the natal cleft may get pushed into the skin pits because of local pressure. Once hair fragments become ‘stuck’ in the skin they irritate it and cause inflammation. Inflamed skin quickly becomes infected and so a recurring or persistent infection tends to develop in the affected area. This kind of infection causes the sinus to develop which often contains broken pieces of hair. Who gets pilonidal sinus? Certain factors increase the risk of developing the condition and include: A job involving a lot of sitting (a sedentary occupation) Being overweight (obesity) A previous persistent irritation or injury to the affected area Having a hairy, deep natal cleft A family history of the condition What are the symptoms of pilonidal sinus? Rapid-onset (acute) symptoms The patient notices increasing discomfort and swelling over a number of days and the pain may be severe by the time of presentation. On examination there is a localised fluctuant swelling in the midline of the natal cleft with overlying cellulitis. The area is exquisitely painful to touch and often simply the act of separating the buttocks to examine the area is intolerable for the patient. Persistent (chronic) symptoms This is common for patients to present with chronic pain and discharge, often with a history of up to two years. On examination a single, or occasionally, multiple sinuses may be seen. Tufts of hair or other debris, such as clothing fibres, are often visible arising from the sinus. Localised edema, swelling and inflammation may be present masking the underlying sinus. What is the treatment for pilonidal sinus? If you have no symptoms If you have no symptoms then you should clear the affected area of hairs by shaving or other means of hair removal. Also, keep the area clean with proper personal hygiene. If you have rapid-onset (acute) symptoms If you have an infection then you should take antibiotics. Painkillers (such as paracetamol and/or ibuprofen) may be very helpful to relieve the pain. Maybe you will need to have an emergency operation to puncture (incise) and drain the ball of pus with surrounding skin infection (abscess). 3. If you have persistent (chronic) symptoms Generally, an operation is recommended.. The options include the following: Wide excision and healing by secondary intention. This operation consists of cutting out the sinus but also cutting out a wide margin of skin which surrounds the sinus. The wound is left open to heal by natural healing processes (healing by ‘secondary intention’). This usually requires several weeks of regular dressing changes until it heals completely. The advantage of this method is that all inflamed tissue is removed and the chance of the condition coming back (recurrence) is low. Excision and primary closure. This means removing the section of skin which contains the sinus. This is done by cutting out an oval-shaped (ellipse) flap of skin either side of the sinus, which takes out the sinus, and stitching together the two sides of the ellipse. After this the wound heals very The risk of a recurrence or of developing a wound infection after the operation is higher than the above procedure. A plastic surgery technique. Sometimes, when the sinus recurs or is extensive, plastic surgery may be advised to remove the sinus and refashion the nearby skin. After any operation Surgeons recommend keeping the area free of hair growing by shaving regularly, or by other methods to remove the hair. This prevents the coming back (recurring) of the problem.

I want to accept like everybody else that Nigeria is broke. Even Mr President said so. The concern in every economic discuss is how the government can generate enough revenue to carry out its campaign promises. A lot of people are suggesting increase in VAT and other taxes etc. It is on this that I want to make my usual controversial analogy. Let us take our mind back to our various villages, which are typically closed economy with minimal external influence. In this my village there is a crop farmer, cattle farmer, fish farmer, teacher, tailor, cobbler, blacksmith, laundry man and a craftsman. Every month, the craftsman makes a beautiful crafts which he takes to the city to sell at the rate of N1,000.00. When he comes back, he would ration the N1,000.00 he made for foodstuff of N200, meat of N200.00, fish N100, pay his children school fees N100, buy shoe N200 and pay for the maintenance of his instrument N200. Ask yourself, what actually determine what the prices he pays for those items? It is what he has. Right? The fish man equally sold fish to the all the households and in return pay to collect their foodstuff, shoes, clothes, maintenance and education for his children. This applies to every households in the village. Can you imagine the value of services and exchange that that N1,000.00 was doing in the village? It is possible for that N1,000.00 to have generated over N100,000.00 worth of exchange. Eventually, the N1,000.00 got to the hand of the tailor who had to go to the city to buy clothing material. He came back with the materials and started sewing. The artisan took his crafts to the city but there was no buyer. The artisan came back to the village with his crafts but without money. The tailor finish sewing but there is no money for his customers to come and collect their clothes. He in turn cannot get his foodstuff, fish or pay his children school fees because he has not receive payment for his work. What do you think would be the implication of this? Now if there is no money in the village, the implication is that all the workers are not likely to be able to exchange their surpluses for what they need and businesses will crumble. Please note that it is not only the artisan that is working. It is just that it is only the artisan whose product has external values. However, if the artisan fails to sell his product, all the villagers would be punished for this; does it make sense at all? That is exactly how the CBN is running Nigeria, every months the CBN, just like that artisan, exchange our crude oil for dollars. But we equally have farmers, teachers, builders, construction workers, doctors, etc who are also engaged in adding value to the economy. The CBN has been unable to quantify the contributions of the people whose products are strictly for local consumption. This is why our informal sector is far bigger than the formal sector because the money in circulation cannot pay for all these services. Then the CBN converts the dollars earned from selling crude oil to our own Naira and give it to us for the payments of every other worker. Then, periodically, it collects back the naira from the tailors/businessmen in exchange for the dollars so that they can import items to the economy. The Naira is then trapped in the CBN vaults until we earned dollars from crude oil. Now that buyers are not interested in our oil, should every other business be suspended because there is no dollars to be converted to naira to pay for them? What are all the naira it collected through sales of dollars and liquidity mopped up kept in its vaults why we are looking to borrow more dollars that would be converted to naira before we can pay our workers? This is crazy, it is voluntary enslavement, isn’t it? The people are working, but there is no money to pay them with simply because the white man refuses to buy our oil!!! What sort of economic sense is that? The Nigerian economy must be reflated. It doesn’t matter whether we are printing new money or the CBN is bringing out money trapped in its vaults. Everybody has accepted the fallacies that our economy is dependent on oil. But is that right? Does it mean that all the above village workers are not contributing to the economy simply because their products are consumed internally? The entire economy of that village has to be regulated by the N1,000.00 the artisan earned. That N1,000.00 determines prices of the foodstuff, fish, clothing, teaching and everything the villagers have to pay for. Any attempt to tax any of them will only shrink the economy further, wouldn’t it? |
