First year medical school in Nigeria is very different. Americans first have to obtain a first degree before medical school, then proceed for a 4 years Med. School.

In Nigeria we do 6 years straight. This article has no relevance to Nigerian medical schools/ students.
The standard text book for pathology is not Robbins (that's for US schools) and our first years don't do H&P until 4th.
They don't read "First Aid - textbook", don't interact with nurses, they can't even relate to this article.

An article that provides realistic, experience expectations and survival strategy is what is more appropriate. Advice on how to stay on target, pass their basic science courses, socialize, develop good study habits.

Copy and paste with no relevance to the intended readers; and gbam it makes front page.

You started a bit well, but your comment on Robbins shows you are as uninformed as the original writer about Nigerian medicine.

Robbins is the Bible of Pathology in Nigeria. Actually, medical textbooks used in Nigeria are exactly same as in US, from Keith More Anatomy, to Robbins Pathology, to Katzung Pharmacology to Gannongs Physiology, Harpers Biochem, Davidsons Medicine and Bailey n Love Surgery. Only Last's Anatomy and Ten Teachers Obstetrics and Gynae are UK texts used commonly in Nigeria.

To a great extent, the points by the OP apply to Naija med students too. Minor exceptions are that we do 6 years straight and not 4 yrs. However, students with 1st degree in related courses do it in 5yrs in nigeria. And our first year is basically basic general sciences, with real medical sciences begining from 2nd year.

Hi,

The minimum time that should elapse before you try to conceive is 6 months, but the longer the better the chances, and of course, the pregnancy would be managed in a tertiary centre.

As for me, there was no case at all. Cos i just went for scan and discovered that my baby does not have any heartbeat and it was confirmed dead. The doctor said he can't see any cause of the fetus death. They induced me so i can expel but my cervix didn't dilate, i was in labor for 3days and nothing on the 4th day they carried out a DnC, but the contraction continues and the pain doubled, so the referred me to do a scan and Xray. When the result came out it shows that majority of the fetus was still remaining and was not in a good position. So they settled for a surgery because i couldn't go through the pain again. They said it was a complicated issue and all through that period i was semi conscious.

She is certainly just past menopause or around it. It is unethical and not the best for her should I prescribe for her, since I have not seen her. She may see a gynae, rather than a GP.

I now understand the circumstances around your operation. It is indeed a perculiar case. I hope you ruled out gestational diabetes as the cause of the intrauterine foetal death, so that you would be more watchful in subsequent pregnancies.

I cannot really say for sure. Have you been ttc? If you have been ttc, and you have no other reason to explain it, the fibroids could be the cause.

Fibroids interfere with fertility when they distort the endometrium (which is not the case with you according to the scan) or when they press on and occlude the tubes (which cannot certainly be seen on scans).

Another reason to worry is when they start to give you potentially serious problems, such as heavy menses or pelvic pains.

Firstly... My mum always complains that her head and her back are always burning her (internal burning sensational). She had gone to many hospitals, and she has done many scans and x ray but they didn't find anything. But d last doctor she visit said its because she has ulcer. He gave her some drugs and d pain reduced but later it started again. please if you have any idea of what might stop it completely i'll be very happy, cos d pain is too much for her.

Secondly... I just had a CS a month ago, after much complication cos my baby died in me at 25wks old. After the CS my stitches opened and it didt heal normally again cos up till now am still going for dressing cos its not fully covered. Please i'll like to know how long i will have to wait before i get pregnant again.

How old is your mom? If she is around or past her menopause, the most likely diagnosis is menopausal symptoms, specifically "Hot flushes". There are prescription medicines that will help, and the symptoms will improve with time.

You should give a 2-year gap between this CS and your next baby, meaning you should start trying to conceive after about a year from the operation. The reason is to prevent rupture of your womb in your next pregnancy/labour.The longer you wait, the stronger the scar from the CS and the better your chances of delivering vaginally next time without complications.

I am curious to know why you were operated on when the fetus was known to have been dead. Or was it a case of placenta praevia with uncontrollable bleeding and dead fetus?

Both results showed you have multiple large fibroids in your womb.

The second scan showed significant free fluid in the pouch of Douglas, suggesting a PID.

The standard practice is to induce at 41W3D.

The more you wait, the more the risks. The risk increases everyday. The risks include a bigger baby, a weaker baby, a less resilient baby or a dead one! It may not happen, but may also happen.

The sensations in your legs are vague and not enough reason to treat you for malaria at 10wks. The sensations are likely to go on their own

Going to lab to check for malaria will almost always have you come back with 1+ or more of malaria parasite. This does not necessarily mean you have malaria.

Decision to treat you for malaria rests on you and your doctor, but drugs used in first trimester include quinine. Many doctors use ACTs like coartem without problems, but this is not recommeneded by any body and it is based on few (inadequate) studies.

You can take avomine (promethazine 25mg as at when needed but not more than thrice a day) for vomitting, if it is much (too many times, makes you weak or dehydrated). Avomine is a category C drugs, meaning it should only be used when its benefits surpass its risks.

Don't you think your observation of the ewcm will be unreliable or difficult, when there is already a whitish discharge?

In any case, the first step is to know if the whitish discharge is normal or not.

It is not related to the delay.

If it continues until your period, and it is cyclical, then it is just part of PMS. Harmless still as regards fertility.

If it is not cyclical, but always there, have a breast exam.

I understand now.

It is normally done on day 21 to comfirm ovulation as well as to assess how prepared your body is for implantation. You should also do one as soon as conceive to ensure that pregnancy levels of your progesterone are adequate.

This discharge you notice, is it always there or only during the time you are supposed to be ovulating?

Does is have a bad odour? Are there any other associated symptoms like pelvic pain?

The cervix is supposed to be closed unless during labour or miscarriage.

If you are not pregnant, there is absolutely nothing to worry about a closed cervix.

Have you done another PT? The first one you did was too early.

You could be pregnant, or it just be the anxiety (ie, if pregnancy is unwanted now.)

I went to general Hospital, so after the various test I was asked to do, the doctor prescribed drugs according to the test results, but after completing the dosage the symptoms still came. I went back to complain but I was asked to buy anti-inflammatory drugs and pain killer. I went back again to complain the same thing happened and I was asked to go carry out the same test. Please if you have some one u can direct me to, I will appreciate it, have spent too much on this disease.

Since you have some relief while you are on the drugs, I believe you should get total relief when the doses are completed after about 2 weeks. Unless it is psychological thing, which will need some counselling.

Treatment for PID should be prescribed by your doctor who must have made the diagnosis too.

I hope you are seeing a doctor?

One would need to know your drug history in order to offer any suggestions. Chronic PID is notoriously challenging to manage. Self or inadequate or stop-and-start treatment obviously reduces patients' chances of cure, as such practices encourage antibiotic resistance.

Is it really spotting (spots of blood on pads, etc) or a brown-like discharge or stain? The latter can be normal after a period, close to a period or with early pregnancy; it does not specifically indicate a problem.

Continued spotting after "normal flow" of 6 days requires investigations.

I have been Ttcing since the evacuation and no BFP yet. I noticed a change in my cycle. I BD 3 times a wk. Most likely, your cycles are not ovulatory (yet), hence the irregularity.

Give it more time; do not be stressed.

Three months is not enough to assume a ttc status in the real sense of the word, esp after a miscarriage. Or were you ttc before you conceived the last time?

For now, you need reassurance and counselling more than drugs and investigations.

Are you self-diagnosing and treating yourself?

Hormone profile to investigate what?

Causes of pelvic pain during sex are many. PID and endometriosis are quite common causes.

Morning,

No doctor would diagnose you with "staph infection", and you should be going to the hospital, not a lab.

It is wrong practice to go to a lab for test and start taking antibiotics on your own. Medicine does not work like that, and it is usually counter-productive and a case of penny wise, pound foolish for patients.

UBTH, Central Hospital and Stella Obasanjo have good doctors at cheaper rates than many private hospitals if finance is the most important consideration.
