Varicocele is commonly and effectively treated in Nigeria by Nigerian doctors. Surgery is the mainstay of its treatment.

Any tertiary hospital (public or private), or any hospital that has a General Surgeon or a Urologist can handle it.

The cost varies widely depending on whether private or public, location and target clients.

It is not a very major surgery. The average cost may not be up to six figures. I do not know a precise amount.

Good morning doctors in the house.
Been ttc for over 2 years.
This cycle, I took parlodel from cycle day 2-21 and also clomid from cycle day 3-7 and was bedding during the fertile window. On the night of cd 20, in the night, felt an intense pulling sensation on the right side of the pelvis for about 10 minutes. On cd 21, had a very light bloody discharge that felt like beginning of AF, from night till following day, it was still not thick blood like menstrual blood, there was bloody discharge on day 22, 23 and 24 although with no blood throughout the day but with fresh red blood at night and light brown by morning. It stopped altogether on cd 25. Did a hpt on cd 26 that gave negative. Did another one this morning cd 31 and still negative. I usually do have a 28 day cycle and btw day 25 and 29, I was having leg cramps, back pain and pain on the sides of boobs. Still no AF. What could this mean doctors?
Your history is not very clear to me.

You hinted you started menstruation on 'cd' 21 thru 25.

Mesntruation marks the start of another cycle, meaning your last cycle lasted 20 days?

First, you should not be depressed or stressed up as this could complicate things.

Doctors generally do not regard an unwanted delay in conception of less than a year as infertility, but still, it helps to ask questions early enough.

First step in investigating a case of infertility is the man's semen analysis. Has dh done this? And how is it?

Hello Dr, am 33, hubby 40. We ve bn ttc for 3yrs+ now. Our last analysis show estradiol <20, Lh 6.5, fsh 5.4, amh 4.27. Also had subserous fibriod which as been laproscopically remove 4 days ago and am recovery well by God's grace. My hubby results show appearance: normal, liquefaction normal, viscosity: 2, concentration: 32, motility:5, nonmotility: 27, progression 1, total motility count 3.5×106 (volume × concept × percent motile). Dr said our best and only option is IVF due to hubby result. And will not advice any treatment for hubby result. He also advice surgery for the fibriod due the size; 10cm before we can start the ivf cycle. My questions please, do we have any other option than IVF? Also we recently purchase fertilaid supplement for for Men and women, motility boost and count, do you think is wise to take these supplements? Dr. Advice we wait 6months before starting the cycle? In short I really appreciate any advice that can improve our situation now cos am so confuse. Thanks so much for your time.

Do not be confused. Be encouraged.

First, your doctor knows you better and he is in the best position to advise you.

Based on the total motile sperm count and your age, IVF is advisable especially if you can afford it.

Removing the fibroid before the IVF is also in order. After that, at least 6 months should elapse before the IVF (or otherwise any attempt at conception) to enable the uterus recover well to carry the fetus.

So your doctor's advice is in order.

IVF may be your best choice, especially if you want to be proactive and can afford it. This does not imply it is the only option, so long as there are motile sperms.

Just that from experience, and being professional, IVF advice is much in order, based on the circumstances.

Generally, healthy diet, healthy lifestyle, weight optimisation, knowledge of one's cycle, stress minimisation, and prayers. Both dh and you.

Hello Doc. I took some contraceptive pills (sendinor) 4months ago, few days after I took it, I had my period (normal) but when it ws time for the next flow, it didn't come so I missed a whole month. The next month after, I saw it and it was also normal (5days). My cycle used to be 28 days, But now, it isn't stable any more cos somtimes its 28 somtimes 30 and now I counted 35 day (all my counts are accurate). though before taking the pill, iv never been bothered about my cycle but now...So please what could be the cause of the unstable/late period? and please do I need to go for some test cos I remembered my 'he' complained of boil on his p_nis few days after intimacy

It is the Sendinor. All hormone-based contraceptives cause irregular menstruation as a side effect.

Your dh should investigate the boil and treat accordingly.

Glucose ++ is an abnormal result, indicative of excess blood sugar.

Do a fasting blood sugar to rule out Diabetes.

Hello Doc. I am having missed period which was supposed to start on d 7th of july. I did home pregnancy test and it showed negative. I was advised to do an infection test cos it do stop menses sometimes. And d result showed (CULTURE: Yeilded Growth of candida alblian) and other things I cudnt read. But my breast is bigger and fuller, and I urinate often I do have cramps like I want to menstruate. Is there a possibility of me being pregnant? Waiting for your reply in anticipation..

If you are pregnant, the PT should have been positive.

Candidiasis will not cause that, and it is easy to treat.

Advice: Do a proper laboratory serum PT. If it remains negative, relax and watch it. If it fails to return after the 7th of August, you will need some investigations.

It may just be that you are too worried.

Thank u so much for d reply. No, d present scan was done in a different diagnostic center. Before taking d supplement d largest was 5cm (that was April) while d present scan showed it's still 5cm with features of degeneration. Before I started d medication d size was increasing almost every other month. Now no more pains, no breakthrough bleeding n less blood loss during my MP. Given dis situation wit multiple fibroids, can implantation occur? Sorry for d many questions, thanks again.
Intramural fibroids seldom cause such problems.

But at 5cm the size is still significant enough to cause problems with implantation, due to distortion of the endometrial cavity

The point is that the size is still large. You should follow it up closely.

PID means pelvic inflammatory disease, which means an infection involving the inner reproductive organs.

Ovulation is the single most important determinant of female fertility.

Let your doctor explain to you more about the specific ovulatory problems you have.

Hello Doc, I had a myomectomy 2011, n d fibroids has since grown back, d largest is 5cm, I've bn on some natural supplements, I went for scan ystday n I was told d fibroids are degenerating. What do u think will happen after degeneration? Will it affect conception? It's multiple intramural fibroid. Thank u
Fibroid degeneration will not affect chances of conception.

Is it the same place you get the supplements that also did the scan for you?

The 5cm, is it the former size of the fibroids or the present size?

Hello Doctor, Really appreciate your response. God bless you. So I went to the hospital and was told to do HVS/Urine M/c/s and pelvic USS. I got the result for my pelvic USS today, waiting on the HVS/Urine result before I see my doctor next...don't know why most of these their handwritings always almost unreadable tho but here is it, " non-gravid anteverted uterus with clear endometrial plate and uniform myometrial echoes. Both adnexae are free. POD contains very significant fluid. Conc: Chronic PID. I understand the PID aspect, please explain the result. Thanks
Every other thing reported is normal. In particular, no evidence of fibroids or cyst, and the lining of the womb is normal

POD containing significant fluid may support a clinical diagnosis of PID.

Somebody I know is suffering from a health condition. He is a farmer. It started when he suddenly fell one day while standing on his own. He could not stand without assistance. An xray revealed he had gaps in his intervertebrate discs. He was treated locally and he felt better. He could stand up and walk around. He fell again and this time it became worse. He probably has what is known as spondylolisthesis, which occurs when a vertebra slips out of place usually a forward shift

The weakness he has is because the vertebra may be pressing on the spinal cord or the nerves controlling movements of the legs. When it gets too serious, it can aftect urination and defecation.

He should stop farming and other physical activities involving his spine for now, and go to a hospital! He will need physiotherapy too, or surgery if the doctor determines that surgery is needed.

He should take NSAIDS for pain if he does not have any reasons not to use nsaids, such as a peptic ulcer. Ibuprofen, diclofenac and piroxicam are cheap readily available nsaids.

You can read more about the condition via google, but avoid self medication.

Hi Doctor, Am having excruciating pain at my lower right abdomen, it happens once in a while around my ovulation period so much that am always too weak to meet with my hubby hence am still TTC for 7months now. Seen a doctor and was given antibiotics then another month, was told to do pelvic scan which I did but they couldn't find anything. Then I have very scanty period flow as well (lasts 2days)...was told to do hormone analysis which I did and tho my progesterone was low, doctor said its still ok to conceive with. I also did home prediction test but negative for ovulation. I honestly don't know what next to do but this pain right now is too much... Please, what do you think could be wrong with me?

Sorry about your pain.

Normally, ovulation is associated with cramps, but this should not be as severe as what you described of the pain.

There are many causes of right lower abdominal pain, but since your pain is cyclical and comes close to the time of your expected ovulation, it may be something about the ovary.

It is also worrying that your ovulation kit test was negative, and that your progesterone assay was low. This predicts anovulation (lack of ovulation).

The advice is to comfirm ovulation as well as investigate the ovaries well through a transvaginal scan. Discuss with your doctor. Also minimise all sources of physical and psychological stress.

In the meantime, minimise the pain with painkillers so that it does not affect copulation when it is most needed.

You should not be worried.

Since you already use an ovulation predictor, what should be worrying is if there are no signs of ovulation.

Ask around you, you will find a scanner.

Read up about scanning and car diagnostics via google.

You can also purchase your own scanning software or device, with as litlle as 3k. Konga has many of them.

Well, the general rule I gave is valid.

The ovulate test kits predict an IMPENDING ovulation. It does this by detecting high levels of the leutnising hormone (LH). The level of the LH increases significantly close to the time of ovulation. It reaches its peak level about 36hours BEFORE ovulation. This event is what the test kits actually predict, which is why they are positive about 2 days to the actual ovulation day. So this explains one part of your observation.

The second part, where you seem to feel ovulation cramps on the 14th day rather than on the 16th day is rather subjective, and probably unreliable.

However, you should know that the most fertile day is not the day of the ovulation. The most fertile days are the 2 - 3 days BEFORE the day of ovulation, meaning that the test kits are still excellent predictors of your most fertile days.

So, your ovulation is still on the 16th day. The test kit is still correct. The test kit is not saying "your ovulation is today". Rather, it says: "your ovulation should occur about 36hours from now, but go meet dh now"

Hi,

For those with a 30 - 31-day menstrual cycle, the ovulation is expected on the 16th to 17th day of the cycle.

The part of the cycle, from the ovulation to menstruation is thought to be constant at 14days. So, usually, if you want to get the ovulation day, all you do is take 14 away from the total number of days in the cycle (cycle length).

Accordingly, in your own case, your cycle length is 30 days. Taking 14 away from this is: 30 - 14 = 16. That is how your ovulation day is expected on the 16th day.

If you next cycle is expected in a week, it may be just too early for a urine PT.

Fasting could result in headaches, due to lowered blood sugar, but stomach aches and tiredness point to a different thing altogether. It could also be Premenstrual Syndrome, if these symptoms are cyclical; ie, if they usually occur a couple of weeks or days to your next period. You should check this up with your doctor.

Do a serum PT a couple of days to your expected date of menstruation, and see.

It could mean that the uterine cavity, the space inside the womb where the baby would grow in, is no longer as free as it should. It is as if something, such as a big fibroid, is pushing on the cavity from outside it, thereby limiting the cavity, or distorting it.

In theory, a cyst could cause irregular periods, but we do not know if it's the cyst that is responsible for your varying cycle length.

Moreso, the variations above are not so much, and could easily have been as a result of an error in calculation.

You should also note that the variation you noticed are still within the normal limits, so you should relax for now.

The advice is to watch it for another couple of cycles, and see what happens.

At 11 weeks, you cannot be feeling your baby's movements, so it couldn't be the baby's movement.

It's hard to imagine any such pulse that could coincide with your breathing.

You should check it up physically with your doctor.

Did you practise exclusive breastfeeding?

Eight months of not having seen your period while still breastfeeding is not alarming, especially if you did or are still doing exclusive breastfeeding.

But note that you could take in before your first period comes, so you should use some contraception if you do not want to risk that chance of pregnancy

Yea, she can mestruate montly even with pcos.

Regular period and lack of ovulation are not mutually exclusive. But lack of ovulation is a common cause of irregular periods.

No hard and fast rules
